Adhoc Student Record Reports

Please note that adhoc student record requests are intended for internal departmental use only. If you need official statistics or information to be used for accreditation, you will need to contact the Office of Institutional Research.

Use the link below to be directed to the login screen for the online data request form. When prompted, login to ServiceNow using your UNT credentials. Complete all required fields and click 'Order Now' to submit your request. PDF forms are no longer required.
