When filling out the application, the questions that have an asterisk next to them are required. The others are optional (in other words, you do not need to list your child's height and weight if you do not want to). When choosing the orchestra level, make your best guess. This can easily be changed once you consult your child and/or your child's teacher. The registration fee is nonrefundable.
