Form	Description
Informed Consent Form - Adults	Consent form, used for most studies, requiring signature.
Informed Consent and Assent Forms - Parents and Minors	Consent and assent forms used for studies enrolling minors.
Informed Consent Form- With HIPAA Authorization	Consent form used when medical information (HIPAA) will be collected.
Informed Consent Notice - Electronic Consent	Consent form used for electronic surveys.
Request for Waiver or Alteration	Required form if you are using the Informed Consent Notice- Electronic Consent form
Recruitment Flyer	A recruitment flyer seeking participation in a human subjects study. All elements of the template must be included but you may format the flyer as you wish.

Studies Submitted Prior to 3/1/2018

Studies that were submitted on paper forms prior to the use of Cayuse, forms are listed below. These are only for studies that were not submitted in Cayuse. Please email completed forms to untirb@unt.edu.

Modification Request	Request to modify an active project.
Renewal Request	Request to renew study, completed typically on an annual basis.
Final Report	Request to terminate an active study.
