Please review the Community Engagement Award FAQs before submitting thisĀ form to answer any questions you may have.

Type the name of the event that you attended. Make sure you type the name of the event as it appears on promotional material.
Enter the date of the event you attended. Remember that you must submit a Campus Engagement Report within two weeks of the event!

Enter the time you arrived/ began participating in the event.

Enter the time the event/ your participation in the event ended.
Please enter the amount of time you were at the event IN MINUTES. (Example: two and half hours = 150.)
Describe the event. Make sure to include the purpose of the event, what the event was, and what you took away from the experience,
