Terms of Agreement Form

This form must be filled out each semester prior to utilizing CVAD IT Services Equipment Checkout

By using this service, you signify your agreement to these Terms of Agreement. If you do not agree to the Terms of Agreement, you may not utilize CVAD IT Services Equipment Checkout. CVAD IT Services reserves the right to modify these Terms of Agreement. Should any changes to the Terms of Agreement be made, you will be required to agree to the updated terms before checking out any equipment.

If you live in a residence hall or temporary housing while at UNT, please also provide your permanent home address.
NOTICE

The Student agrees to retain sole possession of equipment signed for on equipment card(s) and not to lend nor give the equipment to any other person for any reason. The Student is responsible for returning the equipment by the date and time specified. Any equipment not returned within two weeks from the date checked out will be charged to the student at fair market value.

EQUIPMENT IS TO BE USED FOR COURSEWORK ONLY

Any other use will result in disciplinary action. The Student is responsible for any loss or damage to the equipment during the time the equipment is checked out in his/her name, whether such damage is caused by the Student.

LATE RETURNS / ABUSE OF CHECKOUT POLICY
  • 1st offense: incident will be reported to department chair and program coordinator
  • 2nd offense: student will be barred from using UNT equipment checkout services and resources (Student Computing Labs, Fabrication Lab, UNT Libraries, etc.)
  • Final warning: if a student fails to respond to requests to return borrowed equipment, a hold will be placed on the student's UNT account which will prevent class registration, obtaining grades online, etc.
