Advance Care Planning and Texas Directives: More than Document Completion

Needs Statement

This presentation will review the importance of advance healthcare planning and of the discussions needed prior to completing directives. It will also include a brief overview of the main documents currently in use to document advance healthcare plans in Texas.

Target Audience

This activity was planned to meet the educational needs of physicians, physician assistants, nurse practitioners, nurses, and social workers.

Objectives

• Discuss the Institute of Medicine concept of life cycle Advance Care Planning (ACP);
• Describe the importance of beginning ACP discussions with exploring goals of care;
• Demonstrate the importance of asking patients the "living well" question; and
• Summarize the main documents used in Texas for advance care planning
