Needs Statement

To measure improvement, practices must begin tracking metrics.  This eLearning activity will explore the types of metrics health care practices can collect to measure patient satisfaction, quality improvement, and other items of measurement.

Target Audience

This elearning activity is intended for medical professionals (physicians, pharmacists, NPs, PAs, nurses) interested in the Primary Care Medical Home model.  

Objectives

  • Identify metrics that will improve your practice;
  • Define roles for metrics success;
  • Utilize techniques for team engagement; and
  • Verbalize strategies for avoiding pitfalls.
