Needs Statement

The community can include the patient experience so it is necessary, as health care providers, to be aware of the impact community can have on its ability to provide care.  This eLearning activity will explore the role community plays within the health care.

Target Audience

This elearning activity is intended for medical professionals (physicians, pharmacists, NPs, PAs, nurses) interested in the Primary Care Medical Home model.  

Objectives

Identify the importance of talking with patients about community support;

Describe ways the community can add care to the patient, and;

Examine your ability to provide culturally competent care to your patient.
