Needs Statement

In 2003 the Institute for Healthcare Improvement (IHI) developed a tool to monitor adverse event rates in hospitals in an effort to improve patient safety. The trigger tool uses clues to identify adverse effects and helps measure the overall level of harm in the health care organization. This presentation will describe the Trigger Tool and its use.

Target Audience

This activity was planned to meet the educational needs of physicians, nurses, nurse practitioners, physician asssitants, physical therapists and social workers.

Objectives

• Describe the method used with the IHI Global Trigger Tool to detect adverse events in hospital inpatient records
• Summarize the findings of key studies assessing harm in hospitalized patients using the Global Trigger Tool
• Explain why voluntary methods for reporting frequently miss many incidents of harm
