Target Audience

This program was developed for physicians, nurses, social workers and licensed professional counselors.

Objectives

  • Review the Medicare Annual Wellness visit algorithm for assessment of cognition;
  • Review cognitive assessment tools;
  • Apply the Medicare Annual Wellness Visit algorithm to primary care patients; and
  • Demonstrate the Alzheimer's Association app
