This activity is designed for health professionals, including physicians, pharmacists, physician assistances, nurse practitioners, nurses and allied health professionals.

Objectives

Recommend a hypertension treatment and monitoring plan (non-pharmalogical and pharmacological) based on patient specific characteristics.

Identify potential barriers that may prevent achievement of hypertension treatment goals and recommend strategies that can be taken to overcome these barriers.

Discuss the factors that should be considered prior to developing a hypertension telemedicine program.

Discuss the elements that should be documented following an encounter with a patient with hypertension.
