OPERATIONS POLO SHIRT ORDER FORM - The Operations Polo Shirt is a dark blue 100%
cotton shirt with embroidered white block lettering of USCG AUXILIARY on the left side
and the Auxiliarists LAST NAME on the right side. It is available in sizes ranging from
Small (SM) to Triple X (3XL). Orders must indicate a physical delivery address (no post
office boxes) as delivery services is provided through UPS.

OPS Polo Shirt - Moisture Wicking - ORDER FORM - The Operations
Polo Shirt is a dark blue 5.5 oz. 60/40 cotton/polyester mesh (moisture wicking)
shirt with embroidered white block lettering of USCG AUXILIARY on the left side and
the Auxiliarists LAST NAME on the right side. It is available in sizes ranging from
Small (SM) to 5XL for the men's shirt and extra small for the Ladies shirt.
Orders must indicate a physical delivery address (no post office boxes) as delivery
services is provided through UPS.
