For several decades, ISMP has been regularly reporting on preventable medication events associated with high-alert medications and error-prone processes that have resulted in patient harm. Since 2014, ISMP has made a national call for organizations to adopt a number of consensus-based safety strategies called the Targeted Medication Safety Best Practices (TMSBPs) as a means to reduce and even eliminate the processes and conditions that lead to serious patient outcomes.
