Upon request, employers must provide information to state child support agencies about employees, including employment status, information on wages, withholdings, and health insurance.

Respond on the form sent to you or use the Standard Verification of Employment Form, if the state accepts it. States may contact you if they need additional information not provided on the form.

Standard Response to Verification of Employment (VOE) Form
You can use the standard VOE form to respond to state requests for information, if the state child support agency accepts it.
