CDC funds all 50 states, the District of Columbia, and communities to advance the nation’s chronic disease prevention and health promotion efforts. Funded programs include the State Public Health Actions to Prevent and Control Diabetes, Heart Disease, Obesity and Associated Risk Factors and Promote School Health (State Public Health Actions), and the Programs to Reduce Obesity in High Obesity Areas (High Obesity). These two cooperative agreements support CDC’s Division of Nutrition, Physical Activity, and Obesity’s efforts nationwide to make healthy living easier for all Americans through physical activity and healthy nutrition strategies.
