Forms

FVAP is responsible for adopting two key forms that assist voters with the absentee voting process regardless of their State of residence. These forms were amended in 2013 to clarify their usage and requirements after a series of usability studies. The resulting changes to these forms reflect many of the best practices employed by state and local election officials. Below is a description of these forms and their primary purpose. Make sure you have the most up-to-date version of Adobe Reader when downloading the forms.

Federal Post Card Application (FPCA)

Service members, their eligible family members and overseas citizens can use the FPCA to both register to vote and request their absentee ballots.

Federal Write-In Absentee Ballot (FWAB)

The FWAB serves as a backup ballot if the voter does not receive the requested absentee ballot in time. FVAP.gov provides these PDF-fillable forms in addition to online assistants that guide users through the form-completion process.

National Mail Voter Registration Form (NVRF)

The NVRF can be used by military service members, their eligible family members and DoD contractors who are not absent from their voting jurisdiction. All U.S. citizens, including those previously mentioned, can use this form to register to vote, update registration information due to a change of name, make a change of address or register with a political party. The NVRF form is maintained by the Election Assistance Commission.

Envelopes

Voters can mail election materials postage-free using the below envelope. The security envelope is what the voter should seal their completed ballot in to return to the election official. Use these templates and your own, blank envelopes to print with the appropriate labels.
