Addressing HIV Care and Housing Coordination Through Data Integration

The Addressing HIV Care and Housing Coordination Through Data Integration project provides resources and support to integrate data from the Ryan White HIV/AIDS Program and Housing Opportunities for Persons with AIDS (HOPWA) with the goal of improving patient health outcomes. The project is funded by the U.S. Department of Health and Human Services Secretary’s Minority AIDS Initiative, administered by the Health Resources and Services Administration (HRSA).

Cascade AIDS Project (Portland, OR)

Cascade AIDS Project (CAP) is the only HOPWA formula subgrantee in Portland and is a Ryan White (RW) Part A subgrantee. CAP directly enters all client-level housing data into ServicePoint, its HMIS system. ServicePoint is administered locally by the Portland Housing Bureau (PHB) for all service providers in the Multnomah County Homelessness Continuum of Care. Multnomah County Health Department, the RW Part A grantee, maintains CAREWare, the database required of all RW providers in the local TGA.

Information is directly entered by 5 RW providers and imported monthly from CAP's ServicePoint database and a Part-A Clinic that direct enters into EPIC. The TGA service providers work closely with the County’s Research Analysts to maximize the usefulness of the system. For the present project, CAP will add fields to ServicePoint and CAREWare in order to share client housing data with Ryan White providers for the first time. PHB has already supported adding and altering entry fields. The goal of this data integration is to give TGA RW service providers a deeper understanding of how housing and healthcare outcomes interrelate with one another and support enhanced client-centered cooperation. Mapping can be done by CAP staff, which means we can easily work with the County and HMIS to make changes and updates as needed. They will continue to use CAP and County staff to manage the technical aspects of data integration.

City of Hartford (CT) Health & Human Services Department

The City of Hartford Department of Health and Human Services (RWHAP administrator), the City's Central Grants Administration (HOPWA administrator), and AIDS Connecticut (ACT) are collaborating with two case management system developers, jProg, (CAREWare developer for Ryan White) and Nutmeg (CaseWorthy developer for HOPWA), to connect RWHAP and HOPWA providers to increase the capacity of the City of Hartford to use data to improve health.

Gregory House Programs (HI)

Gregory House Programs, the HOPWA subgrantee on the island of Oahu and a Ryan White subrecipient, is working with the Hawaii Department of Health and Case Worthy to build develop a HOPWA database that will be accessible in the existing e2Hawaii platform. Gregory House currently does not house a data management system; instead, each program keeps separate records in Excel. These records will be used as the platform for creating a larger module that will house information for all of Gregory House's clients while providing client-level data in e2Hawaii.

Kansas City (MO) Health Department

Kansas City Health Department (KCHD) will integrate housing data from their HMIS system (MAACLink) into their Ryan White system (SCOUT). Currently, HOPWA providers can access both SCOUT and MAACLink but work primarily in MAACLink. RW users only have access to SCOUT. This project will allow RW users to see housing data from MAACLink in SCOUT.

Palm Beach County (FL) Department of Community Services

Palm Beach County Department of Community Services (DCS) – a Ryan White Part A recipient – will integrate data from their Provide Enterprise (PE) system with data from the City of Palm Beach (the HOPWA grantee). HOPWA providers currently use ClientTrack for data management. Groupware Technologies, the PE developer and project vendor, will integrate ClientTrack into PE and provide access to all RW and HOPWA service providers.

Neither of the partnering agencies provides direct client service: the County funds 6 RW providers and the City funds two HOPWA providers. DCS plans to hire a Data Analyst/Evaluation Specialist to facilitate data integration. Care across the county is coordinated by the CARE Council, who will also be actively engaged in the project.
