PEPFAR

PEPFARThe U.S. President's Emergency Plan for AIDS Relief (PEPFAR) is the U.S. Government initiative to help save the lives of those suffering from HIV/AIDS around the world.  This historic commitment is the largest by any nation to combat a single disease internationally, and PEPFAR investments also help alleviate suffering from other diseases across the global health spectrum.  PEPFAR is driven by a shared responsibility among donor and partner nations and others to make smart investments to save lives.  

The Office of the U.S. Global AIDS Coordinator manages PEPFAR. The Coordinator is appointed by the President and confirmed by the Senate to coordinate and oversee the U.S. global response to HIV/AIDS. Reporting directly to the Secretary of State, the U.S. Global AIDS Coordinator:

  • Leads the U.S. Government's international HIV/AIDS efforts;
  • Ensures program and policy coordination among the relevant USG agencies and departments and nongovernmental organizations, avoiding duplication of effort;
  • Pursues coordination with other countries and international organizations;
  • Resolves policy, program, and funding disputes among the relevant USG agencies and departments;
  • Directly approves all activities of the United States relating to combating HIV/AIDS in 15 focus countries; and
  • Promotes program accountability and monitor progress toward meeting the Emergency Plan's goals.

Global Health Initiative

PEPFAR is the cornerstone and largest component of the U.S. President’s Global Health Initiative (GHI). GHI is the umbrella that links the efforts of U.S. Government health programming into a unified health strategy. GHI pulls together U.S. Government efforts to dramatically reduce the burden of illness and death from some of the world’s most devastating diseases: HIV and AIDS, tuberculosis, malaria, and neglected tropical diseases. GHI has also established ambitious goals to address unacceptable rates of maternal and child mortality. In addition, GHI is working to expand local capacity and self-sufficiency in partner countries. PEPFAR'S Strategy

PEPFAR 3.0 – Controlling the Epidemic: Delivering on the Promise of an AIDS-free Generation documents PEPFAR’s progress and unveils the program’s strategy for working with partners to achieve an AIDS-free generation.  As explained in the report, PEPFAR has moved from phase 1 (2003-2007) – the emergency response phase that brought HIV prevention, treatment and care services to millions to phase 2 (2008-2012) – the sustainability phase, where PEPFAR worked with partner countries among other partners to address the epidemic. The program is now in phase 3 (2013-present) and is focused on transparency and accountability for impact, as well as accelerating core interventions for epidemic control.  PEPFAR is investing resources strategically and geographically to reach populations at greatest risk with evidence-based programs.  The report also provides insight into PEPFAR’s five Action Agendas—the foundation on which an AIDS-free generation will be achieved. The Action Agendas are:

  1. Impact Action Agenda – focusing resources to control the epidemic.
  2. Efficiency Action Agenda – saving lives through smart investments.
  3. Sustainability Action Agenda – sharing responsibility and ensuring maximum coordination with host countries.
  4. Partnership Action Agenda – working with partners to achieve an AIDS-free generation.
  5. Human Rights Action Agenda – securing, protecting, and promoting human rights.

PEPFAR’s Program Results

Annually, PEPFAR reports on results achieved by its programs across the range of prevention, treatment and care programs.  Reflecting America’s commitment to saving lives affected by HIV/AIDS, PEPFAR is supporting countries in providing HIV prevention, treatment and care to their people.

  • As of September 30, 2015, PEPFAR is supporting life-saving antiretroviral treatment for 9.5 million people.  Of these individuals, 5.7 million are receiving direct support. An additional 3.8 million are benefiting from essential technical support to partner countries.
  • As of September 30, 2015, PEPFAR has supported more than 8.9 million voluntary medical male circumcision procedures in Eastern and Southern Africa to reduce the risk of HIV transmission.
  • In Fiscal Year (FY) 2015, PEPFAR provided care and support for more than 5.5 million orphans and vulnerable children.
  • As of September 30, 2015, PEPFAR is supporting training (including pre-service training) for more than 190,000 new health care workers in PEPFAR-supported countries to deliver HIV and other health services.
  • In FY 2015, PEPFAR supported HIV testing and counseling for more than 68.2 million people, providing a critical entry point to prevention, treatment, and care.
  • In FY 2015, PEPFAR supported HIV testing and counseling for more than 14.7 million pregnant women.
    • For 831,500 of the women who tested positive for HIV, PEPFAR provided antiretroviral medications to prevent mother-to-child transmission of the virus.
    • Due to PEPFAR support, 267,000 babies that would otherwise have been infected were born HIV-free in FY 2015 alone
