Instructions for RUS Form 87 (Rev. 5-05)

Request for Mail List Data

This form is used by the Rural Utilities Service (RUS) to obtain current basic information from its borrowers.

Each RUS borrower is required to submit a current RUS Form 87 to the respective Regional/Area Office and to the General Field Representative of RUS each time a change in the information provided on the form occurs.

Fld Name / Item No.	Instruction
Borrower Designation	Enter the 6 digit borrower designation number as assigned by RUS.
Tax Identification Number	Enter the tax identification number of the borrower.
Legal Name	Enter the legal name of the borrower.
Correspondence Address	Enter the address where correspondence is to be sent.
Physical Address	Enter the actual physical address of the office, if this is different than the correspondence address.
Primary E-Mail Address	Enter the primary e-mail address the borrower wishes e-mails to be sent to.
Website URL	Enter the website URL for the borrower, if applicable.
Regular Board Meeting Held On	Enter the day of the month that the regular board meeting is generally held on, i.e. Second Tuesday of Each Month.
Date Set for Next Annual Meeting	Enter the date (mm-dd-yy) that is planned for the next annual meeting.
Phone No.	Enter the main phone number for the borrower, i.e. 123-456-7890
Fax No.	Enter the fax number for the borrower, i.e. 123-456-7890

CORPORATE OFFICIALS

· Enter the name of each of the Borrower’s Corporate Officials and their address. If the title of the official differs from that listed, a different title may listed on the blank “Title” line. If a representative is a Board Member, list “Board Member” on the blank “Title” line.

Name	Enter the Salutation (Miss, Mrs. Mr., etc.,) First, Middle, and Last Name of the Official.
Mailing Address	Enter the mailing address of the Official, include P.O. Box, Street Address, Rural Route, City, State, and Zip Code. Corporate Officials of Electric Borrowers should provide home address. Corporate Officials of Telecommunications Borrowers should provide complete mailing address.

RELATED ORGANIZATIONS

Name of Organization	Enter the name of the organization that represents the borrower. Include the name of a contact at the organization, if applicable.
Mailing Address	Enter the mailing address of the organization, include P.O. Box, Street Address, Rural Route, City, State, and Zip Code. Include a phone number, i.e. 123-456-7890.

The signature of the person who is authorized to sign the form for the borrower is required. The title of the authorized official should also be completed.
