Import/Export Declaration - PDF Format

The Chemical Import/Export Declaration Form is available in PDF format and can be completed online and printed on your printer. Complete Copy 1 (the first two pages of the form ONLY) according to the instructions. Copy 2, Copy 3 and Copy 4 will be automatically completed with the responses given on Copy 1. Additionally, the instructions will provide guidance on how to handle each copy of the form.
