OGE Form 1353: Semiannual Report of Payments Accepted from a Non-Federal Source
Agencies use this form to report, on a semiannual basis, payments for travel, subsistence, and related expenses received from non-Federal sources in connection with the attendance of employees at certain meetings or similar functions. The semiannual reporting periods run from April 1 through September 30 and from October 1 through March 31. Agencies may use either the OGE Form 1353 or the Standard Form (SF) 326.

OGE Form 450-A: Confidential Certificate of No New Interests (Executive Branch)
This form may be used only by those confidential financial disclosure filers who have a previous OGE Form 450 on file with their agencies, who can certify that there have been no changes in their financial interests in the last year, and who work for agencies that allow the use of this form.

OGE Form 450: Confidential Financial Disclosure Report
This form is used by executive branch employees who are less senior than public filers to report their financial interests as well as other interests outside the Government. The purpose of this report is to assist employees and their agencies in avoiding conflicts between duties and private financial interests or affiliations.
