Open the file in your word processor. Note that where you see the italicized and underlined words, you need to replace them with your own agency information. Add the appropriate title for your agency head, have it signed and add your agency logo at the bottom right.

Note: Private sector employers, including private sector contractors working for a Federal Agency, need to obtain OSHA Publication 3165 (English) or OSHA Publication 3167 (Spanish)
