CBSV Enrollment

You must have an Employer Identification Number (EIN) to initiate enrollment. If you do not have one, contact the IRS.

Note:

The complete enrollment process takes up to 6 weeks.

Steps to Enroll

  1. Complete your CBSV Enrollment Form SSA-200 and return it to us by email at ssa.cbsv@ssa.gov.

  2. Send us the one-time non-refundable $5,000 enrollment fee. We accept either a check made payable to the Social Security Administration, or you may complete a Credit Card Payment Form SSA-1414. Mail the fee and a copy of the SSA-200 to:

    Please Note:

    Your submission of the CBSV enrollment form, along with the enrollement fee, constitutes your acknowledgement and agreement that the enrollment fee is non-refundable.


  3. Upon receipt of your enrollment information and the $5,000 enrollment fee, SSA will initiate the formal agreement process by which you and SSA will execute a legally binding User Agreement permitting your participation in CBSV.
