2015 Wolf Creek Dam Open House Tour Registration Form

Please open the Wolf Creek Dam Open House Tour Registration Form. Fill out the document digitally and save it to your computer. Then attach the file to an email and send it to lakecumberland@usace.army.mil to register for a tour. Upon approval of your application, a confirmation email will be sent to you. Please allow for four business days to process the registration. If you have any questions or problems, please call 606-679-6337 from Monday through Friday between the hours of 8 a.m., and 3 p.m. (EST).

Pre-registration is required for all tour guests (including minors) and all tour request forms must be submitted no later than five business days prior to the scheduled tour date.
