All children should receive care within a “medical home;” that is, comprehensive primary care that facilitates partnerships between patients, clinicians, medical staff, and families. This portal, sponsored by the Data Resource Center (DRC) and the National Center for Medical Home Implementation (a cooperative agreement between the Maternal and Child Health Bureau and the American Academy of Pediatrics), is designed to allow state and family leaders quick access to data on how children in each state experience receiving care within a medical home.

The medical home measure on the DRC uses standardized national survey data to assess whether children and youth have a personal doctor or nurse, have a usual source of care, receive needed referrals, receive help coordinating care across multiple provider, and receive care that is family-centered and culturally-sensitive.

The Well-Visit Planner (WVP) is a tool designed by the CAHMI to assist parents in preparing for their young child's (age 0-6 years) well-child check-ups so that they can get the most out of the visit by thinking ahead of time about their questions, concerns, and priorities. Given that one of the sub-components of a medical home is having family-centered care, this is an important tool with many helpful possibilities.

This DRC-developed manual is designed to be a resource for child health program leaders, researchers, and analysts. It looks at methods and findings on measuring medical home from the NSCH and the NS-CSHCN.

The DRC has created a profile for each state using data from the 2009/10 NS-CSHCN which shows the prevalence of having a medical home across various demographic indicators, as well as showing the prevalence of the different components of medical home.
