Medical Home Portal

All children should receive care within a “medical home;” that is, comprehensive primary care that facilitates partnerships between patients, clinicians, medical staff, and families. This portal, sponsored by the Data Resource Center (DRC) and the National Center for Medical Home Implementation (a cooperative agreement between the Maternal and Child Health Bureau and the American Academy of Pediatrics), is designed to allow state and family leaders quick access to data on how children in each state experience receiving care within a medical home.

The medical home measure on the DRC uses standardized national survey data to assess whether children and youth have a personal doctor or nurse, have a usual source of care, receive needed referrals, receive help coordinating care across multiple provider, and receive care that is family-centered and culturally-sensitive.

• Get state-specific data: Get one-page, at-a-glance profiles on how many children in your state meet overall criteria for having a medical home. Additionally, you can interactively search and compare measures by important subgroups of children such as age, sex, race/ethnicity, insurance type and household income.
