Welcome to Health Safety and Work Life Practice Sector Columbia River. The Clinic uses a new schedule structure for planning appointments.  The schedule will incorporate acute, same day appointments, in lieu of the 0730-0830 Sick-Call.  There is no longer Sick-Call.  It is our most sincere hope that this restructuring will alleviate long wait periods that our ill or injured members are currently enduring.  Please understand this in no way reduces the access to care for those members in need of medical attention.

Should you find yourself in need of medical attention please call the clinic front desk at (503) 861-6240 and make an appointment.  If you have an acute illness or injury you will be assigned an appointment time the same day.  If your needs are not acute you will be scheduled as appropriate.  Supervisors, please do not simply send your members to the clinic for ‘sick-call’, but rather call and give your medical staff the opportunity to effectively triage the needs of the member.  As a supervisor you are also authorized to send your member home for 24 hours sick without a duty status chit. If their symptoms persist 24 hours later, this is the point where the member will need to be seen by medical to continue their SIQ status. As always if there is a medical emergency please call 911.

For after-hours assistance (Active duty personnel only) call the Duty HS at (503) 338-8751.  *The Duty HS is for after-hours related healthcare and emergencies only*
If unable to contact the Duty HS call the Sector Columbia River Command Center at (503) 861-6211.

Coast Guard Sector Columbia River Medical/Dental department provides services and support functions for Sector units as well as other local Coast Guard units. The medical facility at the Sector services all active duty personnel within the Sector coastal area of responsibility. It also provides a limited amount of care to reservists. The medical facility is a limited service medical and dental clinic. If the clinic cannot handle the nature of the medical or dental ailment, the patient may be referred to Madigan Army Medical Center in Tacoma, WA., Columbia Memorial Hospital in Astoria, Providence Medical Group in Seaside, or to another local doctor/dentist. In some cases patients may get referred to a TRICARE network doctor in Portland.

In case of an emergency contact 911.

Have Health Questions? Call the TRICARE Nurse Advice Line (NAL)
Sometimes it's hard to know when to seek medical help for urgent health problems. Having access to a trusted medical professional at a moment's notice is invaluable. The new TRICARE Nurse Advice Line (NAL) does just that. Starting April 25, 2014, all TRICARE beneficiaries in the continental United States, Alaska and Hawaii can get health advice by calling the NAL, toll-free and 24/7. The NAL number is 1-800-TRICARE (874-2273).
The NAL is a team of registered nurses who answer urgent healthcare questions. They give beneficiaries professional medical advice to help decide whether self-care is the best option, or they should see a healthcare provider. They will recommend if it's ok to wait for care, or if beneficiaries should seek urgent or emergency care. There is always a live person on the line to answer health questions.
The NAL has pediatric nurses to answer children's health questions. If follow-up to the child's care is needed or requested, the NAL will call back to check on them a few hours later. The NAL can also help beneficiaries find the closest medical care if they need it.

TRICARE ER vs. Urgent Care Visits  If you are considering going to the Emergency Room or Urgent Care, please call the duty HS at (503) 338-8751 prior to doing so. If your injury is a serious emergency, then go straight to the ER and then contact the duty HS. ER visits are covered under TRICARE for Emergencies only. If you are considering going to an Urgent Care clinic for care please be aware that you run the risk of having a medical bill. These visits are not covered under TRICARE without prior authorization either through your PCM or the NAL (please see previous topic). But I just went?! This is still a correctable issue. While this is not the preferred method, on case by case basis you can receive a retroactive referral by visiting your PCM immediately after your visit to an urgent care. The retroactive referral does not guarantee that TRICARE will approve and pay for any medical charges.

The Coast Guard Support Program (previously known as the Employee Assistance Program) provides confidential professional counseling, education, and referral services to you and your family members. Help is available for issues such as marital and family problems, drug abuse, depression and anxiety, work/life balance, work-related concerns and career issues. Confidential help is available 24 hours a day. (CGSUPRT) - 855-247-8778 (855-CGSUPRT) www.CGSUPRT.com

Failed Appointment Policy:
If you miss your scheduled appointment and fail to notify Medical you will be contacted by Clinic staff.  If you miss your second-consecutive appointment your supervisor will be contacted.  If you miss your third-consecutive appointment administrative action will be considered.

All patients must ensure that they bring their Health Record to their appointment if not kept on file within the Clinic.
