During D4 project demolition preparation work on the morning of July 1, 2009, in Hanford’s 300 Area, a millwright fell 50 feet from a catwalk and was severely injured. On the afternoon of October 9, 2008, a sled track test was to be performed involving a sled that consisted of a test package connected to the front of a Super Zuni rocket motor. The purpose of the test was to record thermal battery output during the
impact with the target.

This Report addresses three injuries that occurred on June 15, 2006 during the annual Security Protective Officer Training Competition (SPOTC) at the National Training Center (NTC). One individual required hospitalization for more than a week, much of the time in intensive care. Two other individuals were examined at the hospital and released.

On Tuesday, January 3, 2006, at 1:55 pm, a Bechtel Jacobs Company LLC (BJC) iron worker accidentally fell through a degraded concrete floor panel while working in the K-25 Building at the East Tennessee Technology Park (ETTP).

On January 10, 2006, at approximately 7:47 a.m., a first-line manager (FLM) at the Savannah River National Laboratory (SRNL) received first- and second-degree burns to his head, face, neck, and left hand that required hospitalization after a flash fire occurred during equipment cleaning operations in SRNL Laboratory D-1169.

During the afternoon of June 7, 2005, two postdoctoral (postdoc) employees working at the Los Alamos National Laboratory (LANL) inhaled acid vapors generated by the use of aqua regia (a mixture of hydrochloric and nitric acids) to clean glassware on a laboratory benchtop.

TYPE B Accident Investigation Report of the October 28, 2004 Burn Injuries Sustained During an Office of Secure Transportation Joint Training Exercise at Fort Hunter-Liggett, CA

On October 8, 2004, at approximately 11:00 am, a Grounds Worker at the Pacific Northwest National laboratory (PNNL) fell from a Toro Workman 3200 Utility Vehicle and fracturedhis right leg above the ankle.

On August 25, 2004, an employee of Washington TRU Solution, LLC (WTS) sustained a head injury when he was struck by a C-clamp and rope attachment that broke loose from a piece of metal vent line that was being dragged. This occurred as part of the development and recovery of an existing drift for underground disposal of waste at the Waste Isolation Pilot Plant (WIPP).

On Friday, May 14, 2004, at approximately 11:30 a.m., a dump truck transporting mixing tank T-12 (Tank T-12) from the New Hydrofracture Facility (NHF) Decontamination and Decommissioning (D&D) Project arrived at the Environmental Management Waste Management Facility (EMWMF). Upon arrival, an incoming radiological survey was performed.

Prime contractors need to provide a safe work environment for the entire facility site, including parking lots and outdoor pedestrian walkways. Particular attention needs to be given to areas that must be traversed by individuals with physical handicaps. The contractor must proactively maintain its facilities to ensure a safe work environment for its employees Even minor deficiencies can contribute to significant injury to employees.

On December 17, 2003, at approximately 7:15 a.m., an accident occurred at the U.S. Department of Energy (DOE) East Tennessee Technology Park, Building K-31. An employee (Pipefitter) of British Nuclear Fuels Limited Inc. (BNFL) was injured while attempting to remove concrete block from within a wide-flange, steel column during demolition of the K-31 Control Room (first floor, center of building).

On August 5, 2003, a release of plutonium-238 occurred in a storage room at the Plutonium Facility, Los Alamos National Laboratory, resulting in radiation doses to two workers in the room. The Accident Investigation Board concluded that the direct cause of the accident was the release of airborne contamination from a degraded package that contained cellulose material and plutonium-238 residues.
