COMPASS staff assists member agencies and the public with information requests. Please complete the attached form and submit to COMPASS.

Directions: Document is in Word format. Save the form to your computer, fill it out and then attach it in to an email to Jeanne Urlezaga.

Your request will be forwarded to a COMPASS staff member who will then contact you within two working days to verify that we understand your request and have the resources to respond. If COMPASS staff does not have the resources to complete the request, we will attempt to provide an alternate suggestion as to how or where the request can more likely be completed.
