Effective September 5, 2006, a new Hazardous Waste Manifest form must
be used nationwide by all generators, transporters and treatment/storage/disposal facilities.
(Shipments initiated before September 5, 2006 must use the prior manifest form.)
The Hazardous Waste Manifest Training Video* introduces the new manifest form, highlights the differences between the new and the previous manifest form, and provides specific instructions to generators, transporters and treatment/storage/disposal facilities. It is suggested that you have a printed-paper copy of the new manifest form and the instructions for completing the hazardous waste manifest as you view the training video. Additional information about the new manifest form and federal requirements regarding the management of hazardous waste can be found at the EPA Hazardous Waste Manifest web page. |

Please note that a number of States have additional
State requirements
regarding the use of the new Uniform Hazardous Waste Manifest. Some States require copies to be
submitted to the State, and/or have State-specific waste codes in addition to the federal hazardous waste codes
required to be entered on the new manifest.
