The Office of Inspector General (OIG) maintains a Hotline to facilitate the reporting of allegations of fraud, waste, abuse, or mismanagement in U.S. Department of Energy (DOE) programs or operations.

To submit an allegation to the OIG, complete the form below.

In the box above, please provide a summary of your complaint, to include what, when, where, why and how. For example: ♦ What did the subject do or fail to do that was wrong? Provide specific details regarding the wrongdoing. ♦ When did the alleged wrongdoing occur? ♦ Where did the alleged wrongdoing occur? ♦ What rule, regulation or law do you believe was violated? ♦ How and when did you learn of the suspected wrongdoing? Do you have first hand knowledge of the events? ♦ If alleging contractor or grant fraud, provide details of the contract or grant, to include: contractor, subcontractor, grantee, or subgrantee; type of contract or grant; contract or grant number and/or title; name of agency officials involved in issuing the contract or grant. ♦ Do you have supporting documentation? ♦ If supporting documentation exists outside your control, where can that documentation be found? ♦ Names and contact information for other individuals who are aware of the wrongdoing. ♦ If the complaint is related to the Recovery Act of 2009, please provide information on how the allegations relate to the Recovery Act, and details of the Recovery Act funding.
