	Multiple Civil Enforcement Case Reports	An "I+" icon is displayed if multiple Civil Enforcement Case Reports exist for a facility. Click the “I+” icon to display links to all available Civil Enforcement Case Reports by  case number .  Click on a case number to access the selected Enforcement Case Report.
	Compliance Report	The  Detailed Facility Report  for each facility can be accessed by clicking the "C" icon in the  Reports  column. The Detailed Facility Report presents a concise enforcement and compliance history for a selected facility.
	Effluent Charts	Click the "E" icon to access the facility's Effluent Charts page, which presents dynamic charts, tables, and data downloads of permitted discharges, effluent limits, and violations over time for Clean Water Act (CWA) wastewater discharge permits issued under the National Pollutant Discharge Elimination System (NPDES).
	Pollutant Loading Report	Click the "L" icon to view the detailed pollutant loading report for a facility. This icon links to EPA's  Discharge Monitoring Report (DMR) Pollutant Loading Tool .

• Facilities with Violations in the Last Three Years – Displays the number of facilities in the search results with violations in the last three years based on a count of unique NPDES IDs where Quarters in Noncompliance is greater than zero. 
  Note: This field considers the most current data available, including effluent exceedances reported after the last QNCR was issued; post QNCR data are considered draft and have not been fully quality assured.
• Facilities with Formal Enforcement Actions in the Last Five Years – Displays the number of facilities in the search results with formal enforcement actions in the last five years based on a count of unique NPDES IDs where Formal Enforcement Actions (5 yrs) is greater than zero.
• Facilities with Inspections in the Last Five Years - Displays the number of facilities in the search results with inspections in the last five years based on a count of unique NPDES IDs where Inspections (5 yrs) is greater than zero.

Facility Name  [cwp_name]

Company or permit holder name, as maintained in ICIS-NPDES. The facility name may differ across EPA databases due to differences in reported information, change in ownership, use of parent or subsidiary name, etc.

Street Address (Address, City, County, State, and ZIP Code)  [cwp_street, cwp_city, cwp_county, cwp_state, cwp_zip]

Street address, city, county, state, and ZIP code where facility is located, as maintained by each data system. Certain data systems also maintain mailing address information, which is not used in this report. The street address may differ across EPA databases due to differences in reported information (e.g., use of mailing address), change in ownership, use of parent or subsidiary address, etc. The search logic includes results that match specified address criteria in any source database . The address displayed in the search results table reflects information in the ICIS-NPDES database. 

MSGP Permit Number  [master_external_permit_nmbr]

The general permit that the NPDES ID applies to...

NPDES ID  [source_id]

A unique 9-character ID assigned for each permit within the National Pollutant Discharge Elimination System (NPDES) program. These identifiers are for used tracking purposes in the individual data systems. The ID may contain both letters and numbers and often begins with the two-letter abbreviation for the state in which the facility is permitted.

Mapped  [fac_map_flg]

Graphical indicator denoting whether geographical data required to map a facility are available.

Reports  [Excluded from download]

An indicator of additional ECHO reports that are available and accessible by clicking on the corresponding icons. The Reports Legend provides descriptions of each report icon.

Facility Information (FRS) ID  [registry_id]

Facility Information included on this page is associated with a particular Facility Registry Service (FRS) ID. FRS uniquely identifies a facility by assigning an identification number (FRS ID), and uses this FRS ID to link together all regulatory program database records (such as permit IDs and facility IDs that facilities use in reporting to EPA).

EPA Region  [cwp_epa_region]

The EPA region where the facility is located. EPA has  10 regional offices  that execute programs within several states and territories.

Latitude/Longitude  [fac_lat], [fac_long]

Displays the latitude and longitude at which the facility or permit holder as maintained in the program data.

HUC (Watershed code)  [fac_derived_huc]

The Hydrologic Unit Code (HUC) of the watershed in which the facility resides. A HUC number is assigned to every watershed in the nation and uniquely identifies the watershed.

FIPS Code  [cwp_fips_code]

The Federal Information Processing Standards (FIPS) code to identify the county that a facility is located in.

Federal Agency Code  [fac_federal_agency_code]

Indicates the federal agency, as classified in FRS. Federal agencies are installations that are owned and operated by the U.S. government. The five character code consists of a letter followed by four numbers. There are four possible letters that can occupy the first character position: 'C' for Civilian Federal Agency; 'D' for Department of Defense; 'E' for Department of Energy; 'X' for Unknown. The second and third characters represent the agency code, while the fourth and fifth characters represent the bureau code.

FRS Tribal Land Code [fac_indian_cntry_flg]

Displays “Y” if a facility is flagged as being located in Indian Country, based on information from the EPA's Facility Registry Service (FRS). Displays “N” if a facility is not located in Indian County.

ICIS Tribal Land Flag [cwp_indian_cntry_flg]

Displays “Y” if a facility is flagged as being located in Indian Country, based on information from the EPA's Integrated Compliance Information System (ICIS). Displays “N” if a facility is not located in Indian County.

Within Spatial Tribal Boundary [fac_indian_spatial_flg]

Displays “Y” if a facility is located within 25 miles of a tribal spatial boundary as defined by the U.S. Census Bureau 2010 tribal boundary layer data for tribes in the lower 48 states and Bureau of Land Management Alaska State Office data for tribes in Alaska. Displays “N” if a facility is not located in Indian County.

FRS Spatially Derived Tribe [fac_derived_tribes]

The tribes or tribal territories located within 25 miles of the facility's location. EPA compares the facility location in its Facility Registry Service to the U.S. Census Bureau 2010 tribal boundary layer data for tribes in the lower 48 states and Bureau of Land Management Alaska State Office data for tribes in Alaska. The tribal boundary locations identified are suitable only for general spatial reference and do not necessarily reflect EPA's position on any Indian country locations or boundaries, or the land status of any specific location.

Near US-Mexican Border  [fac_us_mex_border_flg]

A "Y" is displayed for facilities within 100 km of the U.S.-Mexico border.

SIC Code  [sic_codes]

Indicates the facility’s or permit's primary Standard Industrial Classification (SIC) Code. The SIC code describes the primary activity of the facility. The first two digits in the code define a major business sector; the last two digits denote a facility's specialty within the major sector.  SIC Lookup

NAICS Code  [naics_codes]

Indicates the facility’s or permit's primary North American Industry Classification System (NAICS) Code. The NAICS Code has replaced the U.S. Standard Industrial Classification (SIC) system. The system was developed to give special attention to developing production-oriented classifications for (a) new and emerging industries, (b) service industries in general, and (c) industries engaged in the production of advanced technologies.

NAICS industries are identified by a 6-digit code. The first two digits represent the Industry sector, in which there exist 20 broad sectors. The third digit represents industry subsector, the fourth digit represents industry group, the fifth digit represents industry, and the sixth digit is U.S., Canadian, or Mexican National specific. ECHO allows users to search by general industry sector (the two-digit NAICS code) or by industry specialty (the three, four, five, or six-digit NAICS Code).  NAICS Lookup

Discharging to MS4  [dsch_to_ms4]

Displays "Y" if a facility discharges into a Municipal Separate Storm Sewer System (MS4).

MS4 Operator Name  [receiving_ms4_name]

Displays the name of the receiving MS4.

Est. Area Exposed to Stormwater (Acres)  [storm_water_area]

Displays the estimated area of industrial activity at the site exposed to stormwater in acres.

Historic Preservation Criterion  [historic_criterion_code]

Indicates the eligibility criteria for historic properties preservation. The ICIS-NPDES code is provides in parentheses.

• Criterion A (NRB) – The stormwater discharges and allowable non-stormwater discharges do not have the potential to have an effect on historic properties and the facility is not constructing or installing new stormwater control measures on the site that cause subsurface disturbance.
• Criterion B (NSC) – The discharge-related activities (i.e., construction and/or installation of stormwater control measures that involve subsurface disturbance) will not affect historic properties.
• Criterion C (WRA) – The stormwater discharges, allowable non-stormwater discharges, and discharge related activities have the potential to have an effect on historic properties, and the facility consulted with the State Historic Preservation Officer (SHPO), Tribal Historic Preservation Officer (THPO), or other tribal representative regarding measures to mitigate or prevent any adverse effects on historic properties. The facility has either (1) obtained and is in compliance with a written agreement that outlines all such measures, or (2) been unable to reach agreement on such measures.
• Criterion D (CNR) – The facility has contacted the SHPO, THPO, or other tribal representative and EPA in writing informing them that stormwater discharges and allowable non-stormwater discharges have the potential to have an effect on historic properties and the facility did not receive a response from the SHPO, THPO, or tribal representative within 30 days of receiving your letter.

Endangered Species Protection Criterion  [species_critical_habitat_flag]

Indicates the eligibility criteria for the protection of endangered species:

• Criterion A (NON) – No federally listed threatened or endangered species or their designated critical habitat(s) are likely to occur in the "action area" as defined in Appendix A.
• Criterion B (OTH) – The industrial activity's discharges and discharge-related activities were already addressed in another operator's valid certification of eligibility for the action area under this permit, and there is no reason to believe that federally listed species or designated critical habitat not considered in the prior certification may be present or located in the "action area" (e.g., due to a new species listing or critical habitat designation).
• Criterion C (SPU) – Federally listed threatened or endangered species or their designated critical habitat(s) are likely to occur in or near the facility's "action area," and the industrial activity's discharges and discharge-related activities are not likely to adversely affect listed threatened or endangered species or critical habitat.
• Criterion D (SC7) – Consultation between a Federal Agency and the U.S. Fish and Wildlife Service and/or the National Marine Fisheries Service under section 7 of the Endangered Species Act (ESA) has been concluded. Consultations can be either formal or informal, and would have occurred only as a result of a separate federal action (e.g., during application for an individual wastewater discharge permit or the issuance of a wetlands dredge and fill permit), and consultation must have addressed the effects of the industrial activity’s discharges and discharge-related activities on federally listed threatened or endangered species and designated critical habitat. The result of this consultation must be one of the following:
  1. A biological opinion that concludes that the action in question (taking into account the effects of your facility's discharges and discharge-related activities) is not likely to jeopardize the continued existence of listed species, or result in the destruction or adverse modification of critical habitat.
  2. A biological opinion that concludes that the action is likely to jeopardize listed species or to result in the destruction or adverse modification of critical habitat, and any recommended reasonable and prudent alternatives or reasonable and prudent measures are being implemented.
  3. Written concurrence from the applicable Service(s) with a finding that the facility's discharges and discharge-related activities are not likely to adversely affect listed species or critical habitat.
• Criterion E (S10) – The industrial activities are the subject of a permit under section 10 of the ESA, and activities on federally listed species and designated critical habitat.

Stormwater Pollution Prevention Plan  [swpp_url]

A link to the facility's Stormwater Pollution Prevention Plan (SWPPP), where available. A SWPPP identifies the industrial activities conducted at the site, including any structural control practices, which the industrial facility operator will implement to prevent pollutants from making their way into stormwater runoff. Potential Pollutants Exposed to Stormwater  [exposed_activity], [associated_pollutant]

Identifies potential activity and pollutant discharge concerns at the facility, as listed in a SWPPP. For more information, see the EPA factsheets on Industrial Stormwater Guidance.

Control Measure  [control_measure]

Control measures specified in the SWPPP. Identifies the best management practices (BMPs) or other structural or non-structural practices that are used to prevent or reduce the discharge of pollutants in stormwater. For more information, see the EPA factsheets on Industrial Stormwater Guidance.

O&M Schedule  [control_measure_schedule]

The Operations and Maintenance schedule specified in the SWPPP. Identifies the routine practices, such as inspection, training, and maintenance, implemented to keep exposed areas of the site clean. For more information, see the EPA factsheets on Industrial Stormwater Guidance.

Type of Monitoring  [type_of_monitoring]

The type of monitoring specified in the SWPPP. Identifies the type of monitoring activity the facility is required to to conduct based on its industrial activity. Type of Water  [type_of_water]

A unique 10-digit ID assigned for each record/permit/site/facility within the Air Facility System (AFS). These identifiers are for used tracking purposes in the individual data systems.

RCRA IDs  [rcra_ids]

A unique 12-character ID assigned for each record/permit/site/facility within the RCRAInfo database. This identifier is used to track waste handlers subject to the Resource Conservation and Recovery Act (RCRA). The ID may contain both letters and numbers and often begins with the two-letter abbreviation for the state in which the facility is permitted.

SDWA PWS IDs  [sdwa_ids]

A unique 9-character ID assigned for each public water system within the Safe Drinking Water Information System (SDWIS). These identifiers are for used tracking purposes in the individual data systems. The ID may contain both letters and numbers and often begins with the two-letter abbreviation for the state in which the facility is permitted.

TRI IDs  [tri_ids]

A unique 15-character ID assigned for each facility within the Toxics Release Inventory (TRI) program. The format is ZZZZZNNNNNSSSSS, where ZZZZZ = ZIP code, NNNNN = the first 5 consonants of the name, and SSSSS = the first 5 non-blank non-special characters in the street address. These identifiers are for used tracking purposes in the individual data systems.

• Industrial No Exposure Certification (NOE) – Selects facilities with its industrial materials and activities protected by a storm resistant shelter to prevent exposure to rain, snow, snowmelt, and/or runoff is eligible for an exemption from NPDES industrial stormwater permit requirements.
• Notice of Intent – Selects facilities that have submitted a a notice requesting authorization to discharge pollutants to waters of the United States under EPA's NPDES Stormwater Multi-Sector General Permit for industrial stormwater.

Permit Status  [msgp_permit_status]

The code associated with the facility's permit status.

• Active – A complete Notice of Intent (NOI) has been received. The waiting period has been completed and no additional issues have been identified. Coverage under the EPA permit began on the date indicated.
• Expired – The 2008 MSGP expired at midnight on September 29, 2013. A new permit to replace it has not been issued. Facilities that obtained coverage under the 2008 MSGP prior to its expiration are automatically granted an administrative continuance of permit coverage. The administrative continuance will remain in effect until a new permit is issued. Those facilities already covered under the 2008 MSGP are not required to submit a new NOI for permit coverage until the MSGP is reissued, and must continue to comply with all of the requirements in the 2008 permit, including requirements for monitoring and reporting.
• On Hold – A complete NOI has been received, however, it has been placed on hold by EPA. This project or facility is ineligible for coverage under the EPA permit until identified issues are resolved.

Displays the number of instances where effluent limits have been exceeded in the past 3 years (E90 compliance code), based on monthly Discharge Monitoring Reports (DMRs) submitted by facilities.  Note: This field considers the most current data available, including effluent exceedances reported after the last QNCR was issued; post QNCR data are considered draft and have not been fully quality assured.   The number of effluent violations reported in the post QNCR data are added to the total number of officially reported exceedances.

Inspections

Inspections (5 yrs)  [cwp_inspection_count]

The number of inspections/compliance evaluations, under the corresponding statute, occurring at the facility within the last five years. The last five years include data from the twenty most recently completed quarters, plus data from the current quarter up until the refresh date. This count only includes inspection types that are counted as inspections in official counts.

Days Since Last Inspection  [cwp_days_last_inspection]

Indicates the number of days since the most recent inspection of the facility. Inspections listed within the last five years are included.

Date of Last Inspection  [cwp_date_last_inspection]

The date on which the most recent inspection of the facility took place. For the CAA, the date on which a Full Compliance Evaluation (FCE) was completed. This date may or may not correspond to an actual site visit. A series of partial on- or off-site inspections may have been conducted during the fiscal year as part of this an FCE.

Enforcement Actions

Indicates the total number of informal enforcement actions/notices of violations (NOVs) taken against the facility within the last five years. Notices of Violation are activities taken by EPA or the state that often precede a formal administrative or civil/judicial enforcement action. Not all notices of violation are escalated to formal enforcement action for a variety of reasons, including the following: the facility quickly corrects the problem(s) indicated in the notice, the violation is determined to be less severe than originally thought, or consultation between the facility and EPA or the state indicates that a violation has not occurred.

Indicates the total number of enforcement actions taken against the facility within the last five years. This count only includes enforcement actions that have been entered in the national program databases: AFS, ICIS-NPDES, and RCRAInfo. (Please note that all actions at NPDES facilities in states using ICIS-NPDES are counted.)

Local Water Bodies

Impaired Water Body (Section 303(d))  [cwp_imp_water_flg]

Displays "Y" if the permitted facility discharges directly into category 4 or 5 impaired waters, as designated under section 303(d) of the Clean Water Act. Impaired waters are waters that are too polluted or otherwise degraded to meet the water quality standards set by states, territories, or authorized tribes.

 State Waterbody (ICIS)  [cwp_state_water_body_code]

Code from the Assessment TMDL Tracking & Implementation System (ATTAINS) database , assigned by the US Geological Survey, used to classify watersheds in the United States and the Caribbean. The code consists of twelve digits which correspond to six levels of classification:

• Region (first-level, 2-digit HUC)
• Subregion (second-level, 4-digit HUC)
• Accounting unit (third-level, 6-digit HUC)
• Cataloguing unit (fourth-level, 8-digit HUC)
• Watershed (fifth-level, 10-digit HUC)
• Subwatershed (sixth-level, 12-digit HUC)

 State Waterbody Name (ICIS)  [cwp_state_water_body_name]

The name of the watershed in which the facility resides, as entered into ICIS-NPDES. A watershed is an area that drains or contributes water to a particular point, stream, river, lake, or ocean.

Waterbody Most Recent State Assessment  [attains_cycle_year]

The year in which the most recent state assessment occurred for the waterbody. The data are from the ATTAINS database.

Waterbody Name (GNIS)  [rad_gnis_name]

The name of the waterbody from the Geographic Names Information System (GNIS) database in which the facility is permitted to discharge directly. A waterbody is a geographically defined portion of navigable waters, waters of the contiguous zone, and ocean waters under the jurisdiction of the United States, including segments of rivers, streams, lakes, wetlands, coastal waters and ocean waters.

GNIS, developed by the U.S. Geological Survey in cooperation with the U.S. Board on Geographic Names, contains information about physical and cultural geographic features in the United States and associated areas, both current and historical (not including roads and highways). The database holds the Federally recognized name of each feature and defines the location of the feature by state, county, USGS topographic map, and geographic coordinates.

Waterbody Number (Reach Code)  [rad_reachcode]

A nationally unique and permanent identifier for the waterbody, assigned by the US Geological Survey.

 12-Digit WBD HUC (RAD) [rad_wbd_huc12]

Code from the Reach Address Database (RAD), assigned by the US Geological Survey, used to classify watersheds in the United States and the Caribbean. The code consists of twelve digits which correspond to six levels of classification:

The facility derived name of the waterbody in which the facility is permitted to discharge directly. A waterbody is a geographically defined portion of navigable waters, waters of the contiguous zone, and ocean waters under the jurisdiction of the United States, including segments of rivers, streams, lakes, wetlands, coastal waters and ocean waters.

CWA Report (ATTAINS)  [attains_source]

The CWA Report in which the waterbody data can be found. EPA recommended approach to integrating water quality conditions data submitted by states under Clean Water Act sections 303(d) and 305(b).  EPA guidance provides recommended organization for states' Integrated Report submittals. The data are from the Assessment TMDL Tracking & Implementation System (ATTAINS) database .

Listed for Impairment (ATTAINS)  [attains_imp_water_flg]

Displays “Y” if the waterbody in which the facility resides is listed for impairment. The data are from the ATTAINS database.

Under  section 303(d)  of the CWA, states, territories, and authorized tribes are required to develop lists of impaired waters. These are waters that are too polluted or otherwise degraded to meet the water quality standards set by states, territories, or authorized tribes. The law requires that states establish priority rankings for waters on the lists and develop  Total Maximum Daily Loads (TMDLs) , for these waters. A TMDL is a calculation of the maximum amount of a pollutant that a water body can receive and still safely meet water quality standards. Impairment Class (ATTAINS)  [attains_au_category]

The impairment class or category of the waterbody in which the facility is permitted to discharge directly. The data are from the ATTAINS database

• 5. Impaired - TMDL needed.  Available information indicates that at least one designated use is not being supported and a TMDL is needed (also known as the 303(d) listed waters).
• 4. Impaired - TMDL not needed.  Available information indicates that at least one designated use is not being supported, but a TMDL is not needed (see subcategories).
• 4a. TMDL completed.  A TMDL has been completed, but impairment still exists.
• 4b. TMDL alternative.  Impairment is being addressed by a method other than a TMDL (e.g., stream bank improvements).
• 4c. Non-pollutant causes.  Cause of impairment is not a pollutant (e.g., habitat destruction).

Lists all pollutants or stressors that are causing impairment in the assessed waterbody. EJ Indexes Above 80th Percentile

"Y" indicates facilities located in Census block groups in the 80th or higher national percentile of one of the primary environmental justice (EJ) indexes of EJSCREEN, EPA's screening tool for EJ concerns. EPA screens areas in this way to identify geographic areas that may warrant further consideration or analysis. Note that use of this field does not designate an area as an “EJ community” or “EJ facility.” EJSCREEN provides screening level indicators, not a determination of the existence or absence of EJ concerns. Population Density (3 mi) [ fac_pop_den]

The number of persons per square mile in the profiled area. The ratio of total persons (displayed in the Total Persons field) to total land area (displayed in the Land Area field).

Percent Minority (3 mi) [ fac_percent_minority]

Percent Minority is the percentage of the population of the given area that is considered minority. The field is calculated by subtracting the number of persons who are white (and not of Hispanic origin) from the total persons. This number is then divided by the total persons and multiplied by one hundred to determine the percentage.

Statistics are shown for the area within a 3-mile radius of each facility. The radius is measured from the best available lat/long coordinate (normally from EPA's Locational Reference Tables - LRT file). Users should be aware that the Demographic Profile is based upon the reported latitude and longitude of the facility or permit holder. Surrounding populations and other statistics were estimated by retrieving the data for Census block groups within three miles of each facility.
