DON Revision of PII Breach Reporting Forms

The purpose of this memo is to announce revisions of OPNAV Forms 5211/13 and 5211/14 for reporting the known or suspected loss of PII. These revisions do not change current DON policy for PII breach reporting.
The new forms have been re-designated as SECNAV Form 5211/1 (Initial and Supplemental reports) and SECNAV Form 5211/2 (After Action report). Transition to the new forms occurred June 1, 2016.
