Privacy Act Data Cover Sheet, DD Form 2923 dated September 2010, is provided to aid in the safeguarding of personally identifiable information (PII). Use of the form is considered a best practice.
The Privacy Act Data Cover Sheet should be used when hand carrying PII to a recipient with an official need-to-know. It may also be used to cover documents containing PII when working at your desk. When mailing documents containing PII, double wrap the contents and mark the inner container/envelope with either "FOUO - Privacy Sensitive. Any misuse or unauthorized disclosure may result in both civil or criminal penalties" or attach this cover sheet. The cover sheet could also be attached to the documents themselves inside the container.
