Near Miss (Anonymous) form        
Near Miss reporting is a key component to identifying potential mishaps and implementing corrective actions before a mishap occurs. You can submit a near miss anonymously by clicking on the link above, saving the form to a local or shared drive, filling in the form, saving it again and then either faxing the form to the nearest installation safety office as listed on the form or you can e-mail the form to Region Safety by utilizing the link below and attaching it to the email:

Hazardous Material Authorization Request (HMAR)
The HMAR form is used to add a new item to the Authorized Use List (AUL) for your work center. If the HMAR is for a new item or an existing item for a new process, it will be reviewed by the Safety and Environmental offices for review and approval. If possible attach a copy of the MSDS for the product to the e-mail when sending the HMAR request. Only when the HMAR has been approved will it be added to your AUL. The author of the HMAR will be notified of approval. If for any reason one of the designated personnel disapproves the item, that reviewer will contact the originator to obtain additional information and/or suggest an approved alternate product.
