Family Care Plan

A family care plan is an important document that ensures that your dependent children or family members will be cared for during your absence due to deployment or training.

You are required to have a family care plan if you are primarily responsible for the care of young children, family members who are unable to care for themselves in your absence, or if you are a part of a dual-military couple with children.

A completed family care plan must address the following information:

  • names and contact information of your family’s caregiver(s)
  • financial preparations to ensure your family’s financial security
  • arrangements for your family’s transportation to a new location
  • the name of any parent (who is not the caregiver), along with his/her consent to the family care plan
  • the name of the person who will be responsible for your dependent family members in the event of your death or incapacity

As a member of the Navy, you will need to complete the NAVPERS 1740/6, "Department of the Navy Family Care Certificate." For additional information, contact your installation family center or Military OneSource at 800-342-9647.
