If you have a question about a clinic that is not listed, please check with your PCM. Services vary from one clinic to another within the AFMS. Those services may include:

Alcohol and Drug Abuse Prevention and Treatment (ADAPT)


Location: 2501 Capehart Rd, Bldg 4000 (2nd Floor)

Hours of Operation: Monday - Friday, 7:30 a.m. – 4:30 p.m. We are closed on the 3rd Thursday of every month from 1200-1630 for required staff training.

The primary objective of the ADAPT program is to promote readiness, health and wellness through the prevention and treatment of substance abuse, and to minimize the negative consequences of substance abuse to the individual, family and organization.

ADAPT provides services primarily to active duty military members. Non-active duty TRICARE beneficiaries are eligible for assessment and treatment following TRICARE guidelines for access to care, depending on the capacity & capability of the clinic to properly manage each case.

Services include:

  • Population-based educational outreach
  • Preventative brief counseling
  • Evidenced-based outpatient treatment
  • Referral for inpatient detoxification/treatment

Warning signs of Problematic Substance Use:

  • Negative impact on relationships with friends and/or family
  • Used to cope with stress or mood
  • Decreased work or school performance
  • Loss of memory
  • Health problems
  • Legal problems
  • Drinking more alcohol than planned
  • Frequent hang-overs

Audiology


Location: We are located on the main level, across from Public Health.

Hours of Operation: Monday-Friday 0730-1630
We are closed on the third Thursday each month from 1300-1630 for training.

All active duty patients & active duty dependents are eligible for audiology services at the MTF.

Retirees and dependents of retirees may be deferred to TRICARE network providers. The Retiree Hearing Aid Purchase Plan (RHAPP) is no longer available at the MTF.

Services provided: Audiograms, Acoustic Brainstem Response (ABR) testing, newborn hearing screening, Hearing Aid evaluations for active duty personnel and Hearing Conservation Program.

Bioenvironmental Engineering

Bioenvironmental Engineering provides Offutt Air Force Base with operational health risk assessment expertise to enhance commander decision making and health service support capabilities. We conduct our mission through three avenues, Industrial Hygiene, Readiness, and Environmental Health. In Industrial Hygiene, we manage occupational and environmental health (OEH) programs for 62 industrial work centers and provide OEH support to USSTRATCOM and 557WW. In Readiness, we provide Chemical Biological Radiological and nuclear emergency response services through surveillances, risk assessment, decision support and protective measure recommendations. Environmental Health is monitored through ensuring safe potable/non-potable water through Offutt’s drinking water program.

Chiropractic Clinic

Hours of Operation: The clinic is open Monday – Friday 0730-1630 and closes for lunch from 1200-1300.

The Chiropractic Clinic is available for Active Duty patients only. Chiropractic care is available as a selfreferral. Patients do not require a referral from their PCM to receive care, as long as the symptoms are not the result of accident or known injury

Please arrive 10 minutes early for your appointment. If you have not been seen at our clinic within the past 6 months, you will be scheduled with a longer appointment for re-evaluation.

Dermatology

Dermatology services are available by PCM referral.

Initial shaving waivers must be referred by the PCM, but waiver renewals are available after contacting the clinic for walk-in availability.

Some procedures and services provided by the dermatology clinic are considered elective, semi-elective, or cosmetic and are done only on a space-available basis. The need for appointment availability to provide dermatologic care for medically-necessary and medically-indicated treatments and care will take precedence over appointment availability for elective or cosmetic procedures. Accordingly, there is no guarantee that elective or cosmetic services will be available at any given time, or that they will continue to be available in the future even if a patient has had access to them in the past. All of these types of procedures will be booked only by the dermatology clinic at the discretion of the dermatologist based on availability of time, resources, type of service being requested, UHC policy, and other relevant factors.

Ear, Nose and Throat (ENT)

We are a referral only surgical clinic that specializes in the diagnosis and surgical correction of Ear, Nose and Throat issues. When your Primary Care Provider cannot successfully treat you with medication and/or therapy they will refer you to the ENT clinic.

Ear cleaning only Monday and Wednesday: (0730-12:00 and 13:00-15:00) Please contact clinic to coordinate technician availability on Mondays & Wednesdays. Available for patients age 18+

Sleep Apnea: Patients with possible obstructive sleep apnea need a sleep study before appointment with doctor. ENT doctor will only see adult if sleep study shows need for surgical intervention. Dependents ages 17 & under may be seen without sleep study. **NOTE: Referral is need for initial appointment.

What is the EFMP?
The EFMP is a program that works with military and civilian agencies to provide comprehensive and coordinated medical, educational, special housing accommodations, community support, and personnel services to families with special needs.

What is an Exceptional Family Member?
They are an authorized family member who requires special medical, educational, and/or mental health services based on a diagnosed physical, intellectual, and/or mental health/behavioral disability.

What are the benefits of enrollment in the program?
Enrollment allows assignment managers to consider the documented needs of the Exceptional Family Members. When possible, the active duty service member is assigned to an area where the family member’s needs may be met. This will depend on a valid personnel requirement for the service member’s rank, specialty and eligibility for the tour. Additional services may include information and referrals, advocacy, support groups, respite care, and relocation services. Enrollment does not guarantee special consideration for advanced housing placement, appointments or other benefits that are not routinely available to families.

How does a family member enroll in the program?
The active duty service member or spouse calls the EFMP office at Offutt AFB Clinic at (402) 294-6869 or (402) 232-3716 for information or to schedule an appointment.

You may download and complete DD FORM 2792 Medical Summary, AF FORM 1466, AF FORM 1466D and/or DD FORM 2792-1 Educational Summary from the AF Portal page or http://www.e-publishing.af.mil/. If applicable, bring the child’s IFSP or IEP for completion depending on the enrollment eligibility criteria identified. The enrollment should be updated if there are substantial changes to diagnosis or contact information, and when the service member relocates.

The Air Force Exceptional Family Member Program (EFMP) is designed to provide support to military family members with special needs. All branches of the military offer EFMP, and each Service includes a variety of personnel, medical and family support functions under the EFMP umbrella. Family Health Clinic (FHC)


Location: The Family Health Clinic is located within the 55 Medical Group, Ehrling Bergquist Clinic at 2501 Capehart Road Offutt AFB, NE 68113. The FHC is located between the Women's Health Clinic and Immunizations Clinic in the South Tower of the building.

Walk-in Services:
Walk-in services are for non-urgent issues that may be addressed by a nurse or medical technician using approved protocols. Walk-in services are not provider/PCM appointments.

Walk-in hours are Mon-Fri 0800-1530 for the following services:

  • Routine medication injections
  • Follow-up suture or staple removal
  • Urinary Tract Infection (UTI) symptoms in females over age 18 (urinary frequency, burning or urgency). If accompanied by back/side/flank pain, please book an appointment with your provider. Please note that it can take 1-2 hours to receive your lab results
  • Sore throat in patients 3 and older (sore throat is the primary symptom)
  • There is no "sick call", your supervisor may authorize up to 24 hours sick status at their discretion per AFI 41-210, 4.14.6.
  • Pregnancy Tests, You may walk-in to your PCM clinic for pregnancy testing if you meet the following criteria:
    • Positive home pregnancy test OR more than one week late for menstrual cycle
    • No more than one walk-in pregnancy test will be authorized within a 7 day timeframe
    • Walk-in services are only for qualitative testing (Positive or Negative)
    • Dependent children must be 19 years of age or older -or- have a parent or legal guardian present for pregnancy testing

Disease Management: Our Disease Management Registered Nurses focus on improving the health and well-being of our patients by providing education on diabetes, high blood pressure, high cholesterol & asthma management as well as, nutrition, diet and exercise.

Behavioral Health Optimization Program (BHOP): Your provider or team nurse may refer you to our BHOP for more specific care coordination.

We have a Social Worker on staff for questions, concerns and short-term consultations regarding depression, anxiety, pain disorder, tobacco use, alcohol use, diabetes, obesity, insomnia and chronic pain.

Our Behavioral Healthcare Facilitator (BHCF) Registered Nurse can assist with medication follow-up questions and care coordination with medications for depression, anxiety or PTSD. Additionally, our BHCF provides coaching interventions for patients newly diagnosed with chronic diseases.

Introducing Centering Pregnancy® (http://centeringhealthcare.org/)

Centering is a model of group healthcare, which incorporates three major components: assessment, education, and support. Patients meet with their care provider and other group participants that share a similar due date for usually 90-120 minutes, at regularly scheduled visits over the course of their care. Centering promotes greater patient engagement, personal empowerment and community-building. During these longer appointments, you will still have an individual check-up, but it will also allow for a group setting discussion on concerns, typical occurrences during that trimester & the ability to ask questions or advice from more than one source.

55th Medical Group/University of Nebraska Medical Center (UNMC) Family Medicine Residency (FMR) Program information for prospective residents

The combined 55th Medical Group/University of Nebraska Medical Center (UNMC) Family Medicine Residency (FMR) Program is a three year training program for active duty USAF physicians located at the 55th Medical Group, Offutt AFB, NE. There are eight resident training slots in each year of training to make a maximum of twenty-four active duty USAF residents.

USAF Family Medicine residents are assigned continuity patients at the 55 MDG, and have responsibility of managing these patients throughout their training. The outpatient experience with continuity patients is the core of Family Medicine resident training. Residents are mentored and supervised by ten Family Medicine Residency faculty and one licensed social worker.

We welcome third- and fourth-year medical students to visit us. We are excited to offer you many varied training opportunities, and are eager to show you our unique and diverse Family Medicine Residency program.

General Surgery

The clinic is closed daily from 1145-1245. We are closed on the 3rd Thursday of every month from 1200-1630 for required staff training.

The General Surgery Clinic diagnoses and treats patients who require general surgical procedures. Patients are seen with a referral from their PCM, to include Medicare patients not enrolled in TRICARE, on a space-available basis. Surgeries are performed at the University of Nebraska Medical Center and Bellevue Medical Center.

Immunization Clinic

Hours of Operation: Monday – Friday 0730-1600. We are closed on the third Thursday each month from 1100-1630 for training.

The Immunization Clinic provides immunizations on a walk-in basis (0730-1600) for all eligible beneficiaries. Routine vaccinations, yellow fever and Tuberculin (TB) skin tests are administered during normal duty hours. TB skin tests will not be administered on Thursdays or when 48-72 hour reading window falls on a holiday, down day, or training day.

Smallpox vaccines are administered the first Tuesday of each month at 0730 in the Immunizations Clinic.

Patients who have received vaccines from an off-base provider or clinic are required to bring an official copy of record to the Immunizations clinic before services can be provided.

Internal Medicine

Hours of Operation: The clinic is open Monday – Friday 0730-1630. We are closed on the third Thursday each month from 1300-1630 for training.

Please arrive 15 minutes prior to your appointment start time.

Patients with the following conditions may walk-in Mon-Fri 0800-1530 for

  • Females over age 18 with UTI symptoms
  • Patients with sore throat
  • Suture/staple removal
  • Testosterone injections
  • Wart treatments

Coumadin monitoring services are provided by nursing staff through a referral from your PCM.

Laboratory

The Clinical Laboratory provides hematology, clinical chemistry, special chemistry, limited immunology, urinalysis, and microbiology. Tests not accomplished in-house are referred out to other military and civilian facilities for testing.

Test requests for patients seen at the MTF are entered electronically by the provider. Patients are instructed to report to the laboratory for specimen collection or to set an appointment for collection of those samples requiring patient preparation.

The laboratory does not provide results directly to patients, please contact the ordering provider for results.

The clinical laboratory supports the fight against cancer as a C.W. Bill Young/Department of Defense National Marrow Donor Registration Site.

Overseas PCS: AF members PCS’ing overseas should e-mail their Medical Clearance Memo and Personnel Processing Codes (PPCs) to 55MDG.MedStandards@us.af.mil for processing.

After a medical record review, you will receive an email back with instructions and/or information about any necessary actions you need to take. Failure to include all of the above information delays completion of your paperwork.

All emails will receive a response within 1 business day. Emailing your request provides a tracking mechanism for the member and for Medical Standards and is mandatory.

Mental Health

The Mental Health Clinic offers a wide variety of behavioral health services to support individuals, families, and the military mission. Active duty military members have priority for services.

Dependents and retirees are eligible for assessment and treatment following TRICARE guidelines for access to care, depending on the capacity & capability of the clinic to properly manage each case.

Non-active-duty TRICARE beneficiaries may be seen in the network without a referral/prior authorization for the first 8 appointments per fiscal year (1 Oct - 30 Sept) with a network provider for a medically diagnosed & covered condition. Care access/rules vary by beneficiary type, location, TRICARE program option, so check your specific TRICARE benefits: www.tricare.mil or 1-877-988-WEST (1-877-988-9378) or visit the TRICARE service center.

Ophthalmology


Location: We are located on the first floor in the multi-surgical clinic with ENT and Audiology.

Ophthalmology is a REFERRAL ONLY clinic and DOES NOT perform routine eye exams or aircrew physical examinations.

The Ophthalmology Clinic provides medical treatment for diabetic eye disease, glaucoma, cataracts evaluations (but if surgery is indicated patient will be referred to the network), eyelid and orbit problems, dry eyes, strabismus and other ophthalmic disorders.

Any person over 2 years of age, with a surgical referral, can be referred by their PCM for further evaluation. Parents are asked to report to the clinic only with the child who is the patient. Children may not accompany the parent during testing when the parent is the patient.

For all routine eye appointments, call the central appointment line at 402-232-2273, option 12. Optometry clinic front desk personnel schedule follow-up appointments only.

ACUTE eye conditions should be seen by their PCM first.

If you have had an eye exam within the past year and require any of the following, please walk-in during normal duty hours:

  • Gas mask insert / glasses order
  • Glasses repair and adjustment
  • Color vision testing for flight line driver’s license

Contact Lenses:

If you already wear contact lenses, we can update your prescription on a case by case basis. You must be able to wear or bring your contact lenses along with your prescription to your appointment. Prescribed lenses and solutions will be procured at the patient’s expense at their choice of civilian suppliers.

If you do not already wear contacts, we do not do the initial contact lens fitting.
Exceptions: - Members on active flying status and enrolled in the Aircrew Contact Lens Program will receive an initial contact lens fit and subsequent follow-up care).
- Medical conditions requiring contacts.

Ordering Glasses:

Active duty personnel are eligible for glasses and gas mask inserts ordered through our military ophthalmic laboratories. Retired personnel are eligible for glasses only. A retiree does not require an exam from our clinic to order military spectacles if they have a current prescription from a civilian provider. Dependents in any category are not eligible for military procured eyewear. Prescriptions for eyewear will be given to all patients at the end of their exam for use in the civilian community at the patient's expense. The hours for ordering spectacles are Mon-Fri 7:30-11:30 a.m. and 1:00 – 4:30 p.m.

Refractive Surgery:

Refractive surgery is an elective procedure and is available for Active duty personal only.

If you have had at least 2 full eye exams at least 365 days apart AND a provider at Offutt AFB has already deemed you a candidate within the last 6 months please reference the instructions and forms available at http://go.usa.gov/cGxxx (available only from .mil domains).

Orthopedic/Podiatry Clinics

The clinic is closed daily from 1145-1245. We are closed on the 3rd Thursday of every month from 12:00 – 4:30 p.m. for required staff training.

We are a comprehensive musculoskeletal care clinic that provides both surgical and nonsurgical treatment for various injuries and conditions affecting the bones, joints and muscles. Conditions include sports injuries, fractures, arthritis, conditions affecting the hand, and conditions affecting the foot and ankle.

We are staffed by two orthopaedic surgeons, one podiatrist, and one orthopaedic PA. All orthopaedic surgery procedures are performed at Creighton University Medical Center by our surgeons. Podiatric procedures are performed at Bellevue Medical Center by our podiatrist.

Pediatric Clinic

Patients with the following conditions may walk-in for a Nurse/Tech visit Mon-Fri 0800-1530:

Patients 2mos and older enrolled to Pediatrics with Acute Conditions (new or worsening symptoms within 24-72 hours) may utilize the provider based Acute Walk-In hours. Please call ahead (402)232-2273 to verify current hours, as it may change seasonally.

We recommend booking necessary wellness checkups ahead of time, for infants, Kindergarten/7th grade/new to state mandatory screenings, & annual Sports physicals. Schedules are typically open up to 120 days in advance.

Well Child visits are required at the following ages:

This is designed to assist active duty personnel during assignments/reassignments sponsoring Exceptional Family Members (EFM) with special physical, emotional, developmental, or intellectual needs. The Physical Therapy (PT) department provides:

  • Identification, prevention, remediation, and rehabilitation services for acute or prolonged physical dysfunction.
  • Treatment for physical problems involving muscles, nerves, bones, joints, and associated structures.
  • Treatments include heat, cold, traction, manual therapy, exercises, electrical stimulation, etc. Such therapy is prescribed to help the patient function independently by reducing pain and improving function of the effected body part.

Types and ages of patients served in PT include:

Medical/Dental Appointment Booking

Medical appointments can be booked by calling 402-232-CARE (2273) or by using Tricare Online (TOL). Dental Clinic appointments can be booked by calling 402-232-CARE (2273), Option 35

If you experience dental pain, tooth problems or have oral surgery post-operative concerns after duty hours, call 402-232-CARE (2273), Option 2. Ensure you identify yourself as PRP/AUoF to the on-call dentist.

Medical/Dental Appointment Reporting Protocol

PRAP patients must check-in with the PRAP Clinic prior to all medical appointments and ancillary service visits within the 55 MDG. During the check-in process, you will be given a PRP/AUoF badge in exchange for your driver’s license; so, please bring your license with you.

Upon completion of your appointment or ancillary service visit, you must check-out with the PRAP Clinic to ensure all encounters are reviewed by the Competent Medical Authorities (CMAs) and notifications, if required, are made prior to your departure. Your driver’s license will be returned to you at that time.

PRAP patients do not need to check in or out with the PRAP Clinic for Dental Clinic appointments as the Dental Clinic has its own PRAP Monitors and CMAs.

Walk-ins

PRAP members may walk-in for the following services only:

  • Depo Provera injections
  • Testosterone injections
  • Pregnancy testing
  • Strep throat testing

All other concerns must be addressed via appointment.

Medication pick up

All initial prescriptions received from either the Medical Treatment Facility (MTF) or off-base providers MUST be reviewed by a PRAP Clinic and/or Flight Medicine Clinic CMA.

A stamp indicating PRAP Clinic review will be placed on all prescription pick-up forms. Once this is completed, the form may be taken to the Main Pharmacy to be filled/refilled.

Emergency Room (ER) and Urgent Care Clinic (UCC) Visits

PRAP members must notify their Unit PRAP Monitor prior to reporting to an ER or UCC for treatment. PRAP members must receive an authorization number prior to reporting to a UCC for care. Authorization numbers can be acquired by calling the Nurse Advice Line at 1-800-874-2273 after duty hours.

PRAP members are also required to schedule a follow-up appointment with their PCM after all ER and/or UCC visits. You must bring any prescribed medications to your follow-up appointment.

Off-Base Appointments

Prior to reporting for an off-base appointment, PRAP personnel must obtain a Memorandum for Off-Base Healthcare Provider from their Unit PRAP Monitor. The off-base provider needs to complete the memorandum prior to your departure. The completed memorandum must be turned into the PRP Clinic by COB the next duty day for CMA review and MTF PRAP Monitor notification to Unit PRAP Monitor.

Medical Profiles/Waivers

PRAP members seeking medical profiles or fitness waivers require a medical evaluation. An appointment can be scheduled by calling 402-232-CARE (2273) or by using TOL.

Medical profile extensions may require re-evaluation. Requests for extensions can be made by utilizing MiCare or generating a telephone consult by calling 402-232-CARE (2273). The provider will review your medical history and you will be notified if the profile is extended or if an appointment is required.

Navy PRP members seeking PRT waivers, need to bring their completed Physical Activity Risk Factor Questionnaire (PARFQ) NAVPERS 6110/3P (Revised 02-2016) to their appointment.

Prescription Renewals and Diagnostic Test Results

PRAP members seeking prescription renewals, laboratory study and/or radiology/MRI results should leave their Primary Care Manager (PCM) a MiCare message or generate a telephone consult by calling 402-232-CARE (2273).

DEPLOYMENT HEALTH CLINIC

Processes all military deployers and civilian travel medicine health needs. We assist Guard and Civilian deployers BUT please contact us prior to stopping in. If a member is tasked to deploy please contact your UDM to receive reporting instructions.
Deployment Cell also offers overseas travel recommendations to all active duty and dependent personnel who are traveling out of the country. This includes any necessary medications and immunizations. Please inquire at least 2 weeks prior to overseas travel.

OCCUPATIONAL HEALTH

Audiogram: Public Health performs audiograms for individuals who are assigned to the Hearing Conservation Program or on Flying Status. Audiograms are scheduled through the appointment line at (402) 232-2273, option 12.
Fetal Protection Program: All Air Force employees who test positive for pregnancy must have their workplace exposures evaluated to ensure the mother’s safety and that of her unborn child. Please call 402-232-6438 or 402-232-6364 to initiate this evaluation and begin the pregnancy profiling process (for military members).

Radiology

Hours of Operation: Routine X-ray exams are performed on a walk-in basis, Monday through Friday, 7:30 a.m. – 4:30 p.m. All other imaging exams require scheduling through the radiology department either by phone (402)294-7405, or in person.
We are closed on the 3rd Thursday of every month from 12:00 – 4:30 p.m. for required staff training.

Radiology offers a wide range of services to include, Mammography, Ultrasound, Computed Tomography, Fluoroscopy, Routine X-ray, Bone Density (Dexa) and MRI services.

Health and safety concerns warrant that children (regardless of age) not be allowed to accompany patients to their exams. Children under the age of 10 may not be left alone within the clinic without adult supervision. If daycare arrangements cannot be arranged prior to a patient's appointment, please call the radiology front desk to reschedule your exam or contact your provider for assistance.

MAMMOGRAM SELF-REFERRALS

Female patients should check with their PCM to ensure they are current on mammogram screenings.

To facilitate the mammography screening process, Offutt has a self-referral process for women needing a screening exam without any history of breast disease. Females age 50 + with no breast concerns and a Primary Care Manager (PCM) at the 55th Medical Group can self-refer for annual screening mammograms. To self-refer:

  • Call 402-294-7405 or stop by the Radiology front desk to answer a short questionnaire to see if you qualify.
  • Prior mammogram results are needed before an appointment can be scheduled. The process for obtaining prior mammograms from other facilities can take up to 30 days. Stop by radiology and fill out a request for your prior mammograms.

Note:

  • All patients with an off-base PCM require a prescription.
  • Females and males any age with a breast concern should first see their PCM for an evaluation.

Separation and Retirement Physical (SHPE)


All service members retiring or separating must undergo a Separation History and Physical Examination (SHPE), to include Reserve and National Guard members who have served in active duty status a minimum of 180 days or 30 days in a contingency operation.

The purpose of the exam is to identify medical conditions requiring attention, to document current medical status, and potentially assist with the evaluation of VA disability claims. The exam includes a head to toe physical with a provider, a hearing test for all service members and will include other testing based on identified hazards and medical history.

The SHPE should be COMPLETED approximately 180 days prior to your approved official separation date.

  • To schedule your Separation History and Physical Examination (SHPE), please send a signed, encrypted e-mail to 55MDG.SHPE@us.af.mil.
    • Subject Line: SHPE Interview
    • Text: include name, AFSC, date of separation, date of final out, work phone number, cell phone number and state your intention to file a claim with the VA (yes or no).
    • If you are medically separating or retiring, please annotate that in the “Text” section.
  • Do not schedule an appointment with your PCM through the Appointment Line. The Medical Standards Management Element (MSME) Team will handle coordination of your exam(s) during your visit with them.
  • Electronically complete the DD 2807-1 at https://www.tricareonline.com/portal/page/portal/TricareOnline/Portal.
  • Bring your DD 2807-1 and Voluntary Separation Memo to your scheduled SHPE Interview. Upon review, if the member requires an appointment with their PCM, the MSME staff will assist in getting an appointment scheduled.

What We Do
The Referral Management Center (RMC) will:

  • Verify your eligibility and contact information.
  • Verify your referral is in the system.
  • If services are available in the MTF, we will book your appointment with a specialist at the MTF.
  • If services are not available in the MTF, we will explain the authorization process.

Scheduling in the MTF
• If we have the requested services at the MTF, an RMC technician will schedule you during your visit. If you are unable to schedule during your visit the RMC and Audiocommunicator will call you to schedule. You may also call the RMC at your convenience. After three unsuccessful contact attempts, your consult will be pended for 30 days before being closed.

What’s next?
If you are referred to an off-base civilian specialist, you will receive an email alert (if registered at uhcmilitarywest.com) from UHCM&V that your authorization letter is available for viewing/printing. If not registered, the letter will mailed. The letter will contain:

  • the number of authorized visits
  • approved services
  • specialty provider’s name and contact information
  • approved timeframe for treatment

Schedule your appointment with the specialist or chose a different network provider (same specialty). If you change your authorized specialist and your authorization number begins with ‘7’ you do not need to contact UHCM&V for a new authorization. To find a network approved provider visit www.uhcmilitarywest.com

Once you’ve scheduled your appointment with a network specialist, call us at (402) 294-4010 and let us know when and where your appointment is. We will ensure that the results of that visit are received and sent to your Primary Care Manager (PCM) and filed in your military medical record. If we do not hear from you, our Audiocommunicator system will call you to remind you that we need this information.

Checking the Status of a Pending Network Referral?
You can check the status of a pending network referral by calling UHCM&V at 1-877-988-9378
The hours of operation are 0700-1900 local time. You may also visit: www.uhcmilitarywest.com. Registration is required on the site and takes just a few days to process.

What to take to your network appointment
Your authorized off-base specialist will need copies of all pertinent medical history related to the care you are being referred for.

After you have scheduled your off-base appointment, follow these instructions:

  1. Contact your referring clinic no less than eight (8) business days before your scheduled off-base appointment.
  2. Let them know that you will need copies of pertinent medical documentation (e.g., lab results, x-ray reports, medication list, encounter notes) related to the reason you are being referred (not an entire copy of your medical record).
  3. Your clinic will let you know when to pick up your copies or you may request that they be faxed to the specialist’s office.
  4. Take all paperwork with you to your appointment.
  5. Let the specialist know to fax the results of that visit to (402) 232-6406 so they can be put into your electronic military health record for PCM review.

Women’s Health (OB/GYN)

Gynecology
The Women's Health Clinic provides quality nursing care to eligible women (adolescent to geriatric) throughout the childbearing and menopausal cycle. We strive to teach our patients and their families positive health practices through an understanding of available community and facility resources. We provide preventive health care and the maintenance of a healthy life style in an organized, safe, and caring environment. Care includes, but is not limited to:

  • Birth Control Counseling
  • Cancer screening
  • Dysplasia treatment
  • Infertility Work-ups
  • Mammography Referrals
  • OB/GYN preventive medicine
  • Patient/family education
  • Patient/family OB/GYN concerns
  • Sexually Transmitted Infection Treatment

Women with a positive home pregnancy test or more than 5 days late for their cycle can walk-in to their PCM clinic to request a pregnancy test – no referral is needed from the clinic. Results will be received by PCM team, & patients will receive a call with results from their nurse. Active Duty patients must notify public health within 24 hours of positive result. At the time of notification your team nurse will schedule you for the OB Orientation class and order prenatal vitamins.

Obstetrics
OB Orientation – Group orientation held every Tuesday at 0900 in the third floor classroom at the Clinic. You must schedule this class through the appointment line or Women’s Health Clinic. This class is to inform you of the OB care and services provided by the 55th Medical Group and our community partners, and to provide the information you need to choose whether you wish to receive care through the Pink Team (Women’s Health) or the Blue Team (Family Medicine) throughout your pregnancy.
