Clinics and Services

If you have a question about a clinic that is not listed, please check with your PCM. Services vary from one clinic to another within the AFMS. Those services may include:

Aerospace & Operational Physiology


Mission:

Design and execute proactive programs to successfully counter physiological and human performance threats, to enhance health and safety, and maximize war-fighter readiness and combat effectiveness.

Services:
  • Refresher hypoxia training using the ROBD (Reduced Oxygen Breathing Device)
  • Instrument Refresher Course for Aviators
  • ACCES (Attenuating Custom Communications Earpiece System) mold pouring
  • Fatigue education and evaluation using FAST (Fatigue Avoidance Schedule Tool)
  • Maintenance Resource Management training for MXG
  • TeamSTEPPS (Strategies and Tools to Enhance Performance and Patient Safety) training for MDG
  • Human Factors training for all base personnel. Various human factors affect judgment and/or decision making, which can lead to simple errors, increased or duplicated workloads, mishaps, or even death. Enhancing human performance begins on a physiological level, and performance is degraded by environmental and work-related stressors such as:
    • Sleep and Sleep Loss Crossing Time Zones
    • Heat Stress Communications Problems
    • Situational Awareness Shift Work
    • Risk Management Night Shifts and Night Vision
    • Complacency and Inattention Diet, Nutrition, and Fitness

Our Aerospace & Operational Physiology Training (AOPT) program seeks to bolster squadron/unit mission readiness by reducing potential hazards, errors, and mishaps. We can provide one time or recurring briefing support on human factors-related issues or other applicable topics upon request. We can offer a 5-10 minute mini course appropriate for safety meetings and commander's calls.

Dental Flight

The clinic's primary mission is to provide the best healthcare, strengthening worldwide readiness by prevention and treatment of oral diseases. We strive to ensure readiness, achieve best value, and excellence in all we do.

General Information:

The Dental Squadron is a full service dental clinic providing examinations, cleanings, general dentistry, prosthodontics, periodontics, endodontics, treatment of oral facial pain, and oral surgery to our active duty beneficiaries.

Additional Information:

We are unable to provide routine care for dependents or retirees; dental insurance for dependents is available through www.ucci.com, for retirees enrollment is available at www.deltadental.com or by picking up an enrollment form in our office. Please visit www.tricaredentalprogram.com for further information. Dependents or retirees who need x-rays taken can walk-in during duty hours with their x-ray prescription.

Exceptional Family Member Program (EFMP)


The Air Force Exceptional Family Member Program (EFMP) is designed to provide support to military family members with special needs. All branches of the military offer EFMP, and each Service includes a variety of personnel, medical and family support functions under the EFMP umbrella. Mission:

We provide specialty care to ensure the success and safety of specific operators and missions for aviators, air traffic controllers, weapons systems operators and members working in other special operational duty positions. We also provide operational health care that includes: casualty care and management, routine health care and clinical services for families under any circumstance required of the mission. We ensure that eligible Air Force personnel are afforded optimal benefits through healthy behavior, immunization, chemoprophylaxis and health protective devices as appropriate and we develop and maintain programs to educate and motivate members to utilize tools, techniques and lifestyle to enhance safety and performance. In addition, we employ scientifically sound principles of preventative medicine and health promotion to improve overall physical, psychological and social health and performance of individuals to enhance quality of life, increase effectiveness, and ultimately prolong healthy life.

Immunizations


Hours of Operation: Monday-Thursday: 7:30 a.m. to 11:45 a.m., 1:00 p.m. to 4:00 p.m. Friday: 8:30 a.m. to 11:45 a.m.

Inpatient Care


Visiting Hours: Daily from 10 a.m. to 8 p.m.

We are closed the first Wednesday of every month until 1300 for mandatory training.

Laboratory


Location: 90 Hope Drive, Bldg 6000, 1st Floor Clinical Laboratory, Mountain Home AFB, Idaho
Hours of Operation: 7:30 a.m.- 4:30 p.m.

The Clinical Laboratory Flight is a limited service laboratory performing basic testing within the subspecialties of Chemistry, Hematology, Coagulation, Microbiology, Urinalysis, Blood Bank, and Serology. Specialized tests, histopathology, and cytology specimens are shipped outside the facility to military and civilian reference laboratories. The Clinical Laboratory Flight provides support to MDG outpatient, inpatient clinics and eligible beneficiaries.

Important Notes:

The laboratory does not provide results directly to patients.

Optometry

Patient Administration / Outpatient Records / Release of Information


Location: First Floor, Section A, Building 6000
Hours of Operation: Monday through Friday 7:30 a.m.-4:30 p.m. We conduct training the 1st Wednesday of each month starting at noon. We are closed on Federal Holidays and ACC-directed Down Days.

The Patient Administration Office assists beneficiaries in many ways. We can assist with new patient registrations, Patient Travel Reimbursements, Birth Certificates and Proof of Birth Letters, Virtual Out Processing for TRICARE, Incapacitation requests, Line of Duty Determinations, Medical Evaluations Boards, and educate patients who are enrolled into the Exceptional Family Member Program.

Outpatient Medical Records (TOPA Records) - Provides outpatient medical records to clinics, upon request, for patient appointments. Upon receipt of PCS orders, medical records are packaged and mailed to the member's gaining unit. If a member is retiring or separating, all electronic medical records are printed and filed in the outpatient medical record prior to being mailed off for retirement.

Release of Information - Provides copies of medical records to members, legal, insurance companies etc. as requested. Routine record requests are usually completed in approximately 30 days.

Pharmacy


Location: The Pharmacy is located in building 6000 (366th Medical Group).
Hours of Operation: 7:30 a.m. - 5:00 p.m., Monday - Friday, except for holidays and approved ACC family days. The MDG is closed until 1:00 p.m. the first Wednesday of every month.

To better serve you, the 366 MDG pharmacy is currently upgrading its capabilities. Although we do not stock every drug, our formulary is very robust and is designed to meet the majority of our beneficiaries' prescription needs. We realize that you, as a TRICARE beneficiary, have a choice as to where you have your prescription filled. Our goal is to provide a superior level of customer service so that we become your pharmacy of choice. We look forward to serving you.

Important Notes:

Patients must activate their prescriptions with pharmacy staff prior to prescriptions being filled. Patients can activate prescriptions either in person or over the phone. Hard copy prescriptions presented after 4:30 p.m. will be ready for the following duty day. All refill requests must be called in to the automated refill line 208-828-6947 or toll-free at 1-800-446-8980.

Referral Management

The Referral Management Center (RMC) is where patients go to process specialty referrals requested by their referring provider. The RMC can help set your Military Treatment Facility (MTF) specialty appointment. We can also help coordinate your off-site network provider if necessary. Additionally, the RMC can advise on health care benefits and review referrals.

Important Notes:

If you have been "referred" by your Primary Care Manager, before leaving the clinic, please stop by the RMC. The RMC can assist with appointing you to an MTF specialty clinic for your initial visit. All follow-up appointments to the MTF sub-specialty clinic will be coordinated with that clinic and not the RMC.

If care cannot be provided on base, the RMC will defer you to our one of our TRICARE United Health Care: Military & Veterans specialist medical provider. All routine network referrals have an expiration date, so please set your initial appointment before the referral time period ends. The RMC cannot "renew", "modify", or "delete" an unused, unwanted, or expired referral. All expired referrals must be "renewed" by calling the appointment line at 208-828-7900 and following the tree to your assigned Family Health Clinic and leaving a T-Con for your PCM team.

Duty Hours: For care considered Urgent, please call the 208-828-7900 line for same day appointments to the MTF clinics. If an appointment cannot be made to the MTF, please visit our 24 hour Urgent Care Center located at the back side of the 366th Medical Group.

Non-Duty Hours: Please call the appointment line at 208-828-7900 and speak to the Nurse Advice Line. You can also visit our 24 hour Urgent Care Center located at the back side of the 366th Medical Group.

Emergency Care does not require a referral, but you are strongly advised to contact your PCM within 24 hours of ER/ED release at 208-828-7900. M-F during duty hours.

UCC


24 hours a day seven days a week
(All facilities excluding UCC are closed the first Wednesday of every month for training, ACC Family Days and National Holidays.)

Urgent Care


Staffed ambulance and urgent care center with in-house physician 24-hours a day.
