You may report your concern or comment anonymously, but it may be more difficult to investigate the concern or comment thoroughly if you do not include your name and contact information. One reason why including your name and contact information is important is because sometimes the information you provide is not complete and we may need additional information. Another reason why including your name and contact information is important is because it allows us to follow up with you regarding the results of our investigation should you request that.
