About Incident Reporting

Whom should I inform if I'm involved in an incident where a release or a suspected release of a hazardous material has taken place in transportation?
Hazardous Materials Regulations (49 CFR Parts 171-180) require certain types of hazmat incidents be reported to PHMSA through the Hazardous Materials Incident Report Form DOT F 5800.1

Section 171.15 of the HMR requires an immediate telephonic report (within 12 hours) to the National Response Center following an incident 49 eCFR 171.15 .  You may call the NRC at 1-800-424-8802.

Section 171.16 requires a written report for certain types of hazmat incidents within 30 days of the incident, and a follow-up written report within one year of the incident, based on certain circumstances. 49 eCFR 171.16

What is an Incident Report Form? 

Incident Report Form 5800.1 is a guidance document for preparing incident reports, and tabular summaries of hazardous materials incidents by mode, year, state, hazard class, etc.
