Our patients are at the forefront of a healthcare revolution in primary care. Our vision is to proactively improve your health through the patient-centered care provided by our medical home staff. The medical home model launched here in 2009 has drawn significant interest throughout the military, and is now being implemented throughout the Military Health System (MHS).

We are transforming the role of primary care to support a model where the key prescription is to encourage and impact healthy behaviors. We group our staff of 250 healthcare professionals into medical home teams, where patients receive a collaborative, comprehensive and holistic set of healthcare solutions from the providers on that team. The teams include not only staff internists, but also dieticians, tobacco cessation specialists, psychologists and mind-body therapists. And our suite of technology tools helps improve access so that those patients who truly need to see a provider can—that same day.

We serve active duty and retired military personnel, and including those needing geriatric services. We also provide routine and preventive care, consultative services for patients with complex medical conditions, inpatient care, and acute care (sick call) for active duty military personnel.
