Respondent was assessed a civil penalty of $12,320 for four violations of the HMR when it represented and marked DOT specification compressed gas cylinders as having been successfully hydrostatically retested and requalified for use in accordance with the HMR when it failed to hold a requalification identification number (RIN) (violation No. 1); it had not performed any hydrostatic pressure test on the cylinders (violation No. 2); it failed to maintain records of hydrostatic pressure testing (violation No. 3) and its employee had not received hazmat employee training (violation No. 4).
