Respondent was assessed a civil penalty of $2,450 for two violations of the HMR, when it offered Division 1.4 (explosive) hazardous materials for transportation in commerce which were not accompanied by a hazardous materials shipping paper, and it had not created and retained records of hazmat employee training.
