Please note: After December 12, 2015, this form can ONLY be used to make updates to existing USDOT number registrations, not for initial registration with FMCSA. First-time applicants must use the Unified Registration System (URS). For more information about URS, or to register for the first time, click here.

Warning: The MCS-150C form is for intermodal equipment provider (IEP) registrations and updates only. Forms submitted by non-IEPs will not be processed by FMCSA.

MCS-150C Form Instructions

Each intermodal equipment provider (IEP) that offers intermodal equipment for transportation in interstate commerce must update its records via the Intermodal Equipment Provider Identification Report (Form MCS-150C). 

The USDOT Number serves as a unique identifier when collecting and monitoring a company's safety information acquired during audits, compliance reviews, crash investigations, and inspections. As an intermodal equipment provider, your company's safety information will be acquired through roadability reviews, crash investigations, and inspections.

After FMCSA receives the completed MCS-150C Form, it will generally take 5-7 days for the update to be processed. This estimate does not take into account the extra time required for mail service, incomplete or incorrectly-completed forms, forms, etc.

Updating by Mail:

  • Submit all required forms together
  • Once the application is completed, FMCSA suggests making a copy for the company’s files
  • Mailed applications, on average, take four to six weeks processing time

Note: Applications that are incomplete, unreadable, or unsigned will be rejected and returned via mail, delaying the update of the company’s records.
