Respondent was assessed a civil penalty of $11,100 for five violations of the HMR when it: represented and certified DOT specification and special permit cylinders as qualified for continued service when it failed to conduct the required internal visual inspection (violation No. 1), it had not verified the accuracy of its retest equipment at a test pressure within 500 psi of the pressure at which it retested the cylinders (violation No. 2), it had not notified PHMSA in writing of changes in personnel performing cylinder requalification functions (violation No. 3), it allowed employees to perform functions subject to the requirements in the HMR when they had not received the required initial or recurrent hazmat employee training (violation No. 4), and it failed to maintain complete and accurate records of system calibration and cylinder requalification (violation No. 5).
