Respondent was assessed a civil penalty of $6,435 for five violations of the HMR when it represented, marked, and certified DOT specification compressed gas cylinders as having been successfully retested and qualified for continued use in accordance with the HMR when it had not verified the accuracy of its test equipment as required (violation No. 1); it had not notified PHMSA of a change in its personnel who were testing cylinders (violation No. 2); it had not maintained complete and accurate records of cylinder reinspection and retesting (violation No. 3) or the Compressed Gas Association pamphlets applicable to the cylinders it qualified for continued use (violation No. 4); and its employees had not received security awareness training, and it had not maintained records of hazmat employee training (violation No. 5).
