California Man Charged with Falsely Certifying Oxygen Cylinders for Aircraft

Summary

On May 28, 2015, Danniel Hoose, owner of Shasta Fire Equipment, Inc. was charged in U.S. District Court, Sacramento, California, with falsely certifying oxygen cylinders for re-use in aircraft.

The information alleged that in April 2013, Hoose represented that he performed hydrostatic testing on a special permit oxygen cylinder installed in an aircraft, when, in fact no such testing had been completed. The investigation determined that Hoose’s re-qualification equipment was in disrepair; therefore, he was unable to properly test the cylinders in accordance with Pipeline and Hazardous Materials Safety Administration (PHMSA) regulations. Hoose allegedly falsified the cylinder hydrostatic testing certifications and returned the cylinders to their owners for use.

We are conducting this investigation with assistance from PHMSA.
