Summary

On May 5, 2016, Michael Glenn Dairy pleaded guilty in U.S. District Court, Abilene, TX, to a charge of making a false statement on a U.S. DOT medical form and medical card. Dairy admitted to falsifying a registered nurse’s (RN) signature on the DOT medical form and medical card and submitting them to his employer. 

This investigation was based on information provided by an RN from West Texas Injury Prevention (WTIP) in Big Spring, TX. According to the RN, a representative from OTP Pumping (OTP) contacted her office to obtain her national registry number after an employee, identified as Dairy, submitted a DOT medical form and medical card containing the RN’s signature. The RN advised OTP she did not sign the form and card and she did not give any person(s) her effective consent to forge her name on an official Government document. Subsequently, Dairy was interviewed and admitted to falsifying the medical form and card using the RN’s signature from a previous physical examination that was administered at WTIP.
