INSPECTION TABLE

Data Element	Definition
INSPECTION ID (INSPECTION_ID)	[ Max Field Length 8 ] This is a unique identifying number assigned to an inspection record.
USDOT NUMBER (DOT_NUMBER)	[ Max Field Length 8 ] This is the USDOT number assigned by MCMIS to a census record. Each motor carrier should have only one active census number. The census number has no internal coded structure. Numbers are issued sequentially as entities are added to the system.
REPORT STATE (REPORT_STATE)	[ Max Field Length 2 ] State/District/Province which uploads the inspection, usually where inspection was conducted. (See Appendix A [Opens in New Window] )
REPORT NUMBER (REPORT_ NUMBER)	[ Max Field Length 10 ] A number identifying the inspection report. Format SS########, where SS is the state abbreviation, or US for Federal inspections, followed by 8 alphanumeric.
INSPECTION DATE (INSP_DATE)	[ Max Field Length 8 ] Date the inspection was conducted. (YYYYMMDD)
START TIME (INSP_START_TIME)	[ Max Field Length 4 ] Time the inspection started. Format: HHMM (military time) where HH is the hour and MM is the minutes. Midnight is recorded as 2400. 9999 = Time unknown
END TIME (INSP_END_TIME)	[ Max Field Length 4 ] Time the inspection ended. Format: HHMM (military time) where HH is the hour and MM is the minutes. Midnight is recorded as 2400. 9999 = Time unknown
REGISTRATION DATE (REGISTRATION_DATE)	[ Max Field Length 8 ] Indicates the date the certification of correction of defects was registered.
REGION (REGION)	[ Max Field Length 2 ] FHWA/FMCSA region in which the inspection took place. (1, 3-10) Region 1: CT, ME, MA, NH, NY, NJ, RI, VT, PR and VI Region 3: DE, DC, MD, PA, VA and WV Region 4: AL, GA, FL, MS, NC, SC, TN and KY Region 5: IL, IN, MI, OH, MN and WI Region 6: AR, LA, NM, OK and TX Region 7: IA, KS, MO and NE Region 8: CO, MT, ND, SD, UT and WY Region 9: AZ, CA, NV, HI, GU and AS Region 10: AK, ID, OR and WA
STATUS CODE (CI_STATUS_CODE)	[ Max Field Length 1 ] Indicates the status of the inspection. (U = Unprocessed, T = To Census Search, C = Complete, N = Non-match, I = Intrastate, H = FMCSA hold, P = Potential Resolution, X = Non-motor carrier)
SAFETY INSPECTOR KEY (SAFETY_INSPECTOR_KEY)	[ Max Field Length 6 ] Code identifying the inspector. Each state has its own format.
LOCATION (LOCATION)	[ Max Field Length 6 ] State assigned code representing the physical location of the inspection.
LOCATION DESCRIPTION (LOCATION_DESC)	[ Max Field Length 30 ] Physical location where the inspection took place.
COUNTY CODE STATE (COUNTY_CODE_STATE)	[ Max Field Length 2 ] State/District/Province of the county where the inspection took place. (See Appendix A [Opens in New Window] )
COUNTY CODE (COUNTY_CODE)	[ Max Field Length 3 ] FIPS code identifying the county in which the inspection occurred. (See County Code State [Opens in New Window] )
INSPECTION LEVEL ID (INSP_LEVEL_ID)	[ Max Field Length 8 ] This field identifies the level of the inspection using the following code: 1 = Full 2 = Walk-Around 3 = Driver-Only 4 = Special Study 5 = Terminal 99 = Invalid
SERVICE CENTER (SERVICE_CENTER)	[ Max Field Length 2 ] Service Center Region the inspection occurred in. (MW = Midwest, EA = East, SH = South, WE = West.
CENSUS SOURCE ID (CENSUS_SOURCE_ID)	[ Max Field Length 8 ] USDOT NUMBER verification method. (1 = Matchware, 2 = Manual Census Search, 3 = Matching Algorithm, 4 = Pen-Based (not used on Crashes), 5 = SafetyNet Uploaded and not changed, Null = Unknown)
INSPECTION FACILITY (INSP_FACILITY)	[ Max Field Length 1 ] Type of facility where the inspection was conducted. (F = Fixed, R = Roadside)
SHIPPER NAME(SHIPPER_NAME)	[ Max Field Length 120 ] Shipper name.
SHIPPING PAPER NUMBER (SHIPPING_PAPER_NUMBER)	[ Max Field Length 15 ] Shipping paper document number.
CARGO TANK (CARGO_TANK)	[ Max Field Length 3 ] Three-character indicator of the cargo tank type. Comes from ASPEN.
HAZMAT INSPECTION TYPE (HAZMAT_INSP_TYPE)	[ Max Field Length 1 ] Indicates the type of Hazardous Materials Inspection. (B = Bulk, N = Non-Bulk, Null = Not Applicable)
HAZMAT PLACARD REQUIRED (HAZMAT_PLACARD_REQ)	[ Max Field Length 1 ] Indicates if hazardous material placards are required. This field is required if a hazardous materials code is entered. (Y = Yes, N = No, Null = Unknown)
ASPEN VERSION NUMBER (ASPEN_VERSION_NUMBER)	[ Max Field Length 5 ] ASPEN version number.
SAFETYNET VERSION NUMBER (SNET_VERSION_NUMBER)	[ Max Field Length 2 ] The SAFETYNET system version number.(Can be blank.)
SAFETYNET SEARCH DATE (SNET_SEARCH_DATE)	[ Max Field Length 8 ] The date the "Carrier Search database" was created. The database is used by SAFETYNET to verify USDOT NUMBERS.
ALCOHOL/CONTROLLED SUBSTANCE CHECK (ALCOHOL_CONTROL_SUB)	[ Max field length 1 ] Indicates whether an alcohol or controlled substance check was performed. (Y = Yes, N = No, Null = Unknown)
DRUG INTERDICTION SEARCH (DRUG_INTRDCTN_SEARCH)	[ Max field length 1 ] Indicates whether or not there was a drug interdiction search. (Y = Yes, N = No, Null = Unknown)
DRUG INTERDICTION ARREST (DRUG_INTRDCTN_ARRESTS)	[ Max field length 2 ] Indicates the number of drug interdiction arrests made.
SIZE WEIGHT ENFORCEMENT (SIZE_WEIGHT_ENF)	[ Max Field Length 1 ] Indicates whether the inspection was conducted in conjunction with a size/weight enforcement action. (Y = Yes, N = No, Null = Unknown)
TRAFFIC ENFORCEMENT (TRAFFIC_ENF)	[ Max Field Length 1 ] Indicates whether the inspection was conducted in conjunction with a traffic enforcement action. (Y = Yes, N = No, Null = Unknown)
LOCAL ENFORCEMENT JURISDICTION (LOCAL_ENF_JURISDICTION)	[ Max Field Length 1 ] Indicates whether the inspection was conducted by a local jurisdiction. (Y = Yes, N = No, Null = Unknown)
INSPECTOR CONFIDENCE LEVEL (INSP_CONFIDENCE_LEVEL)	[ Max Field Length 1 ] Indicates the Inspector's level of confidence on the carrier / USDOT number match. Could be X, O or Null
PEN CEN MATCH (PEN_CEN_MATCH)	[ Max Field Length 1 ] Indicates use of ASPEN carrier match. Could be B, I, N, P, Q, Y, or Null
PEN FIELD 2 (PENFIELD2)	[ Max Field Length 1 ] Reserved for ASPEN data.
PEN FIELD 3 (PENFIELD3)	[ Max Field Length 1 ] Reserved for ASPEN data. Could contain 0-9, E, N, or Null
FINAL STATUS DATE (FINAL_STATUS_DATE)	[ Max Field Length 8 ] Date the Status Code is set to 'C', 'N', 'H' or 'I'. (See STATUS CODE above.)
POST ACCIDENT INDICATOR (POST_ACC_IND)	[ Max Field Length 1 ] Indicates whether or not the inspection was conducted after an accident. (Y = Yes, N = No, Null = Unknown)
GROSS COMBINED VEHICLE WEIGHT (GROSS_COMB_VEH_WT)	[ Max Field Length 6 ] Indicates the total gross weight of the combination vehicle. (Measured in pounds.)
DEFECT VERIFICATION (DEFECT_VER)	[ Max Field Length 1 ] This field is no longer used. It may contain historical data.
OUT-OF-SERVICE DEFECT VERIFICATION (OOS_DEFECT_VER)	[ Max Field Length 1 ] A code indicating whether or not any actions were taken to correct out of service defects. A-Repaired at scene B-Towed, escorted to a repair service C-Restricted (no longer used) D-Other U-Unknown/unverified N-Non-OOS or Driver violation
TOTAL HAZMAT SENT (TOTAL_HAZMAT_SENT)	[ Max Field Length 1 ] A count of the number of associated insp hazmat records.
VIOLATIONS NOT SENT (VIOL_NOT_SENT)	[ Max Field Length 2 ] Indicates the total number of violations that were not uploaded from the state. (Historical)
OUT-OF-SERVICE NOT SENT (OOS_NOT_SENT)	[ Max Field Length 2 ] Indicates the total number of out-of-service violations that were not uploaded from the state. (Historical)
TOTAL VIOLATIONS (VIOL_TOTAL)	[ Max Field Length 3 ] Indicates the total number of violations for the inspection.
TOTAL OUT-OF-SERVICE VIOLATIONS (OOS_TOTAL)	[ Max Field Length 3 ] Indicates the total number of out-of-service violations for the inspection.
TOTAL DRIVER VIOLATIONS (DRIVER_VIOL_TOTAL)	[ Max Field Length 3 ] Indicates the total number of driver violations for the inspection.
TOTAL DRIVER OUT-OF-SERVICE (DRIVER_OOS_TOTAL)	[ Max Field Length 3 ] Indicates the total number of driver out-of-service violations for the inspection.
TOTAL VEHICLE VIOLATIONS (VEHICLE_VIOL_TOTAL)	[ Max Field Length 3 ] Indicates the total number of vehicle violations for the inspection.
TOTAL VEHICLE OUT-OF-SERVICE (VEHICLE_OOS_TOTAL)	[ Max Field Length 3 ] Indicates the total number of vehicle out-of-service violations for the inspection.
HAZMAT VIOLATIONS TOTAL (HAZMAT_VIOL_TOTAL)	[ Max Field Length 3 ] Indicates the total number of hazardous material violations for the inspection.
HAZMAT OUT OF SERVICE TOTAL (HAZMAT_OOS_TOTAL)	[ Max Field Length 3 ] Indicates the total number of hazardous material out-of-service violations for the inspection.
SAFETYNET SEQUENCE ID (SNET_SEQUENCE_ID)	[ Max Field Length 10 ] This is an uploaded field and is the Safetynet 2000 database primary key for the record. We store it and send it back with any Safetynet 2000 record that has to go back to the state for any reason.
ORIGINAL REPORT STATE (ORIG_REPORT_STATE)	[ Max Field Length 2 ] Original report state.
ORIGINAL REPORT NUMBER (ORIG_REPORT_NUMBER)	[ Max Field Length 10 ] Original report number.
ORIGINAL INSPECTION DATE (ORIG_REPORT_DATE)	[ Max Field Length 8 ] Original Inspection date.
ORIGINAL START TIME (ORIG_REPORT_TIME)	[ Max Field Length 4 ] Original start time of the inspection.
TRANSACTION CODE (TRANSACTION_CODE)	[ Max field length 1 ] Reason for upload from SAFETYNET. (A = Add to MCMIS, C = Change existing record in MCMIS)
TRANSACTION DATE (TRANSACTION_DATE)	[ Max field length 8 ] Date the inspection record was entered/changed in SAFETYNET. (YYYYMMDD)
UPLOAD DATE (UPLOAD_DATE)	[ Max Field Length 8 ] Indicates the date that the inspection was uploaded. (YYYYMMDD)
UPLOAD_FIRST_BYTE (UPLOAD_DOT_NUMBER)	[ Max Field Length 8 ] Census number uploaded from the state. Could be a temporary state assigned number.
UPLOAD SEARCH INDICATOR (UPLOAD_SEARCH_INDICATOR)	[ Max Field Length 1 ] This is an uploaded field used to tell if SAFETYNET used Matchware to determine the uploaded DOT-Number.
CENSUS SEARCH DATE (CENSUS_SEARCH_DATE)	[ Max Field Length 8 ] The date record is extracted from database and sent through the Census Search process.
MCMIS ADD DATE (MCMIS_ADD_DATE)	[ Max Field Length 8 ] Date the inspection was added to MCMIS. (YYYYMMDD)
CHANGE BY USER (CHANGE_BY_USER)	[ Max Field Length 30 ] The user who made the most recent change to this record.
CHANGE DATE (CHANGE_DATE)	[ Max Field Length 8 ] Indicates the date that the inspection record was last changed. (YYYYMMDD)
CHANGED BY APPLICATION (CHANGE_BY_APPL)	[ Max Field Length 61 ] The application that added or updated the record.
SAFETYNET INPUT DATE (SNET_INPUT_DATE)	[ Max Field Length 8 ] The date the inspection was input into SAFETYNET.
SOURCE OFFICE (SOURCE_OFFICE)	[ Max Field Length 7 ] The office that sent the inspection.

CARRIER ID (INSP_CARRIER_ID)	[ Max Field length 8 ] This is the primary key in the INSPECTION CARRIER Table; it uniquely identifies a carrier.
INSPECTION ID (INSPECTION_ID)	[ Max Field length 8 ] This is a unique identifying number assigned to an inspection record.
CARRIER NAME (INSP_CARRIER_NAME)	[ Max field length 120 ] The name of the individual, partnership, or corporation responsible for the transportation of goods or persons.
CARRIER STREET (INSP_CARRIER_STREET)	[ Max field length 50 ] The street address of the principal place of business of the carrier reported under CARRIER NAME.
CARRIER CITY (INSP_CARRIER_CITY)	[ Max field length 25 ] The name of the municipality (city or township) of the principal place of business of the carrier reported under carrier name.
CARRIER STATE (INSP_CARRIER_STATE)	[ Max field length 2 ] State/District/Province of the principal place of business of the carrier reported under CARRIER NAME. (See Appendix A [Opens in New Window] )
CARRIER ZIP (INSP_CARRIER_ZIP_CODE)	[ Max field length 10 ] ZIP code of the principal place of business of the carrier reported under CARRIER NAME.
COLONIA (INSP_COLONIA)	[ Max Field length 25 ] This field specifies the carrier's Mexican Colonia. (For Mexican domiciled Carriers)
PREFIX (PREFIX)	[ Max Field length 2 ] Docket Number Prefix (usually MC, MX, or FF).
DOCKET NUMBER (DOCKET_NUMBER)	[ Max Field length 8 ] The docket number (MC, MX, or FF number) assigned by FMCSA or the former Interstate Commerce Commission to the carrier reported under CARRIER NAME. Formerly known as the ICC number.
INTERSTATE (INSP_INTERSTATE)	[ Max Field length 1 ] Indicates whether the carrier is involved in interstate commerce. (Y = Yes, N = No)
CARRIER STATE ID (INSP_CARRIER_STATE_ID)	[ Max Field length 12 ] Identification number assigned by the state to the carrier.

DRIVER TYPE DRIVER_TYPE	[ Max Field Length 1 ] Indicates the type of driver. P = Primary, S = Secondary
DRIVER LAST NAME DRIVER_LAST_NAME	[ Max field length 20 ] The last name of the driver of the vehicle.
DRIVER FIRST NAME DRIVER_FIRST_NAME	[ Max field length 20 ] The first name of the driver of the vehicle.
DRIVER MIDDLE INITIAL DRIVER_MI	[ Max field length 1 ] The middle initial of the driver of the vehicle.
DRIVER DATE OF BIRTH DRIVER_DATE_OF_BIRTH	[ Max field length 8 ] The date of birth of the driver. (YYYYMMDD)
DRIVER LICENSE STATE DRIVER_LICENSE_STATE	[ Max field length 25 ] The license state/district/province of the driver. (See Appendix A [Opens in New Window] )
DRIVER LICENSE NUMBER DRIVER_LICENSE_NUMBER	[ Max field length 2 ] The driver's license number.

UNIT NUMBER (INSP_UNIT_NUMBER)	[ Max field length 1 ] The sequence number of the vehicle unit. (1-6)
UNIT MAKE (INSP_UNIT_MAKE)	[ Max field length 10 ] The make of the vehicle unit.
UNIT COMPANY NUMBER (INSP_UNIT_COMPANY)	[ Max field length 15 ] The number assigned to the vehicle unit by the carrier.
UNIT LICENSE (INSP_UNIT_LICENSE)	[ Max field length 12 ] The license tag number on the vehicle unit.
UNIT LICENSE STATE (INSP_UNIT_LICENSE_STATE)	[ Max field length 2 ] The state/district/province issuing the license tags on the vehicle unit. See Appendix A [Opens in New Window] )
UNIT VEHICLE ID NUMBER (INSP_UNIT_VEHICLE_ID_NUMBER)	[ Max field length 17 ] The vehicle identification number of the unit.
DECAL ISSUED (INSP_UNIT_DECAL)	[ Max field length 1 ] Indicates whether a CVSA decal was issued. (Y = Yes, N = No, U = Unknown)
UNIT DECAL NUMBER (INSP_UNIT_DECAL_NUMBER)	[ Max field length 8 ] The number on the CVSA decal.

VIOLATION ID (INSP_VIOLATION_ID)	[ Max field length 8 ] This is a unique identifying number assigned to a violation.
INSPECTION ID (INSPECTION_ID)	[ Max field length 8 ] This is a unique identifying number assigned to an inspection record.
VIOLATION SEQUENCE NUMBER (SEQ_NO)	[ Max field length 3 ] The sequence number of the violation.
VIOLATION CODE PART NUMBER (PART_NO)	[ Max field length 3 ] Part number of the violation cited. (Reference FMCSR, HMR)
VIOLATION CODE SECTION NUMBER (PART_NO_SECTION)	[ Max field length 40 ] ] Section number of the violation cited. (Reference FMCSR, HMR)
VIOLATION UNIT (INSP_VIOL_UNIT)	[ Max field length 1 ] Driver, co-driver, or vehicle unit # which had the violation. (D, C, 1-6)
UNIT ID (INSP_UNIT_ID)	[ Max field length 8 ] This is a unique identifying number assigned to an inspected unit.
VIOLATION CATEGORY (INSP_VIOLATION_CATEGORY_ID)	[ Max field length 8 ] The category number assigned to the violation. (1-40) (See Appendix C [Opens in New Window] )
OUT OF SERVICE INDICATOR (OUT_OF_SERVICE_INDICATOR)	[ Max field length 1 ] Indicates whether the violation resulted in an out-of-service. (Y = Yes or N = No)
DEFECT VERIFICATION ID (DEFECT_VERIFICATION_ID)	1 = A   -Repaired at scene 2 = B   -Towed/Escorted to repair service 3 = N   -Non-OOS/Driver Non-OOS 4 = D   -Other 99 = U   -Unknown/Unverified
CITATION NUMBER (CITATION_NUMBER)	[ Max field length 15 ] The number identifying the citation
CITATION RESULT (CITATION_RESULT)	[ Max field length 2 ] The code that is given by the state to indicate the result. This defaults to 0 (zero) for “N/A”.
CITATION_ISSUED (CITATIONN_ISSUED)	[ Max field length 2 ] Is an indicator that an actual citation was issued, not a warning.

The above columns will be on EVERY violation for EVERY inspection, regardless of a citation issued or adjudicated.

INSPECTION VIOLATION ID (INSP_VIOLATION_ID)	[ Max field length 8 ] A unique number that relates a particular violation to a particular inspection.
SHIPPER NAME (SHIPPER_NAME)	[ Max field length 120 ] The shipper's name.
SHIPPER STREET ADDRESS (SHIPPER_STREET)	[ Max field length 50 ] The shipper's street address.
SHIPPER CITY (SHIPPER_CITY)	[ Max field length 25 ] The shipper's city.
SHIPPER STATE (SHIPPER_STATE)	[ Max field length 2 ] The shipper's state/district/province. (See Appendix A [Opens in New Window] )
SHIPPER ZIP CODE (SHIPPER_ZIP_CODE)	[ Max field length 10 ] The shipper's ZIP code.

SEQUENCE NUMBER (SEQ_NO)	[ Max field length 3 ] This is the sequence number of the special study. The number indicates whether data is in Study field 1, 2, 3, 4, or 5.
