How to Order File Extracts

  1. Print the MCMIS View File Extract Order FormFile Extract Order Form. [Opens in New Window]
  2. Follow the instructions below to fill out the form.
  3. You can call FMCSA at (800)832-5660 with any questions.
  4. Send order form and check to FMCSA.
Order Form Instructions

  • Select File Extract & Format
  • Place an "X" in the box corresponding to the File Extract(s) you wish to order. This order form may be used to order:
    • Census File: you do not need to specify a year.
    • Crash File: please specify the year or years you wish included in the space provided (up to three years may be chosen). Currently, FHWA State-reported crash data are available from years 1989 to present. If you do not indicate which year(s) should be included, the CD-ROM will contain the most recent calendar year available.
    • Inspection File: please specify the year you want extracted. (Inspection File Extracts are only available one year per extract.) If you do not indicate which year you want, the CD-ROM will contain the most recent calendar year available.
  • Provide Your Mailing Address
  • In the space provided, tell us where to send the output. Be sure to include the name and telephone number (including area code) of a person we can contact should we have questions about your order.
  • Calculate Cost
    • Enter the costs for each file extract ordered.
    • Add the costs of the extracts to determine the total cost of your order.
    • Enter the total cost in the box labeled "Amount of Check."
  • Send order form and payment to:

If you would like to order a CSP, follow the directions below.

  • Follow the instructions below to fill out the form.
  • Send order form and check to FMCSA.
  • Order Form Instructions

    • Specify Company Information
      • In the space provided, enter the full name and complete address of the company to be profiled. The company’s USDOT number and/or MC (ICC) number are requested, and will help us to process your order accurately.
      • If you wish to order more than one Company Safety Profile (CSP), provide complete information for all additional companies on separate sheets of paper.

      • *Note: CSPs are available to the public under the Freedom of Information Act (FOIA). However, certain information in the CSP, namely Driver Data, contains private information that is not covered by FOIA and will not be included in a CSP that is disseminated to the public. Of course, a company may have access to its own Driver Data. For this reason, Driver Data will only be released to those who are registered as authorized recipients of that information.
    • Provide Your Mailing Address: In the space provided, tell us where to send the CSP. Be sure to include the name and telephone number (including area code) of a person we can contact should we have questions about your order.
    • Calculate Cost: CSPs cost $27.50 each. See Price List. [Opens in a New Window]
    • You may, however, discount the total cost of your order (see below). To be eligible for the discount, all CSPs ordered must be shipped at the same time and to a single location:
      • 15% when ordering 2-6 CSPs
      • 20% when ordering 7-12 CSPs
      • 25% when ordering 13 or more CSPs
    • You may also order subscriptions with this form. Reports will be sent out on a yearly basis (once a month) for $330.00, quarterly for $110.00, and semi-annually for $55.00. There is no discount on subscriptions.
    • Determine the total cost of your order by following these steps:
      • Multiply the number of CSPs ordered times $27.50.
      • Enter the result in the "Initial Cost" box.
      • If you are ordering:
        • 1 CSP, enter "0" in the "Discount" box.
        • 2-6 CSPs multiply "Initial Cost" by 0.15; enter the result in the "Discount" box.
        • 7-12 CSPs multiply "Initial Cost" by 0.20; enter the result in the "Discount" box.
        • 13 or more CSPs multiply "Initial Cost" by 0.25; enter the result in the "Discount" box.
    • Subtract "Discount" from "Initial Cost"; enter the result in the "Amount of Check" box.
    • Select the medium on which you wish to receive the CSP by placing an "X" in the appropriate box. CSPs are available on the following media:
      • Printed report.
      • CD-ROM

    • Select Report Type
      • Indicate the type of Personalized Report you wish to order. To select the report type desired, mark an "X" in the appropriate box. Mark only one box.
        • If you select the Personalized Crash Report or Inspection Report, specify the calendar year(s) to be covered by the report. Currently, FHWA State-reported crashes are available for the years 1989 - present. If you do not specify a year, the report will cover the most recent calendar years available.
        • If you choose the Personalized Census Report, you do not need to specify a year.
        • If you select the Personalized Inspection Report, specify the calendar year to be covered by the report (only one year can be specified). Currently, Inspection data is available dating back to 1989. If you do not specify a year, the report will cover the most recent calendar year available.
      • Select Additional Data Elements
        • The core elements always provided in Personalized Reports are listed below.
        • If you wish, you may include up to four additional data elements in your report. Select additional elements for Personalized Census Reports from the Census File Elements, for Personalized Crash Reports from the State-Reported Crash File Elements, and for Personalized Inspection Reports from the Inspection File Elements.

        • Write the names of any additional elements you select in the spaces provided in Section A of the order form.

      • Describe Companies, Crashes, or Inspections to be included
        In the space provided describe which companies, crashes, or inspections should be included in your report.
      • For example, do you want all companies in the MCMIS Census File or only companies in the state of Maryland? All crashes reported in 1994, or only crashes that occurred in Texas during 1994?
      • You must base your description on data available in the MCMIS Census File (in the case of a Personalized Census Report), or the State-Reported Crash File (from 1989 to the present.) For example, you cannot ask for a Personalized Census Report covering all vehicles that are the color "yellow" because color data are not available in the Census File. You can, however, ask for a report covering all carriers operating one hundred or more vehicles, since fleet size is one of the data elements in the Census File.
      • In describing the companies, or crashes, you wish to include, you may use up to four Census File Elements.
        In describing the crashes you wish to include, you may use up to four Crash File Elements.
        In describing the Inspections you wish to include, you may use up to four Inspection File Elements.

      • In the following examples, the words in bold are data elements or their values.
      • For example: "Include all companies located in the State of Maryland," or "Include all private companies whose current safety rating is conditional," or "Include all crashes which occurred in Texas during daylight where the driver appeared to have been drinking."

      • How the companies, crashes or inspections should be sorted:
      • Sorting refers to the order in which companies/crashes/inspections will be displayed in your report. By placing an "X" in the appropriate box, you may select the standard sort format or specify your own format.

        • Standard Sort Format: Companies/Crashes are grouped by state and listed alphabetically by company name within each state.
        • Other Format: If you choose to specify your own sort format, describe how the companies/crashes should be sorted in the space provided. You may use up to four data elements from the MCMIS Census File or State-Reported Crash File in your description. The following examples illustrate how the order in which you list these elements affects the appearance of the report. The words in bold are data elements.
        • For Example:
      If you write in Section C:
      "Sort the carriers by company classification, state, and Company legal name."

        The cost of a Personalized Census Report is $33.00, a Personalized Crash Report is $33.00, and a Personalized Inspection Report is $100.00. A check/money order for that amount must be mailed with the order form.

        • Select Report Type Indicate the type of Count Report you wish to order. To select the report type desired, mark an "X" in the appropriate box. Mark only one box.
          • Census Count Report:you do not need to specify a year.
          • Crash Count Report:specify the calendar year(s) to be covered by the report. (You may choose up to three years in a single Crash Count Report.) Currently, FHWA State-reported crashes are available from years 1989 to present. If you do not specify a year, the report will cover only the most recent calendar years available
          • Inspection Count: please indicate the year you want the report to cover. (Only one year may be selected for the Inspection Count Report.) If you do not specify a year to be covered, you will receive the most recent available year.

For example, you cannot ask for a Census Count Report covering all vehicles that are the color "yellow" because color data are not available in the Census File. You can, however, ask for a report covering all carriers operating one hundred or more vehicles, since fleet size is one of the data elements in the Census File.
