SYSTEMS INTEGRITY SAFETY PROGRAM (SISP) FINAL RECOMMENDATION REPORT

PURPOSE

On date, (partner) and the Pipeline and Hazardous Materials Safety Administration entered into a Systems Integrity Safety Program Agreement to enhance and improve safety and security outcomes thru stakeholder partnerships and to ensure compliance with 49 CFR Parts 171-180 (Hazardous Materials Regulations or HMR). Partner has taken corrective action to address all probable violations noted during the course of the program. The recommendations provided in this report are designed to provide a level of safety in excess of that required under the HMR.

FINAL REMARKS

Please note that all recommendations listed above are non-binding. These recommendations are suggestions meant to provide a level of safety in excess of that required by the HMR and to help to reduce the number ofhazmat incidents in transportation. By following these recommendations, PHNISAanticipates that Partner will avoid future noncompliance through continuous improvement and quality assurance programs. PHNISA hopes that Partner will continue to make the public safety a high priority.

This letter officially concludes the review which was being conducted by the Office of Hazardous Materials Enforcement. As agreed, there will be no enforcement action taken for any probable violations noted by PHNISA during the term of the Agreement, as Partner took immediate action to rectify these violations. Please be aware that any future violations of the HMR will be subject to enforcement action.
